上尿路结石手术留置双J管型号选择的临床体会 黄绍明,周小青，张国辉，赖吉安，杨斌，殷青辉，曾祥泰，李才胜， 【摘要】 目的：探讨输尿管镜钬激光碎石手术留置不同型号双J管对碎石排出的影响。方法：对232例输尿管结石患者行经尿道输尿管镜钬激光碎石手术，术中采用经尿道输尿管镜留置不同型号的双J管并观察术后碎石的排石情况。结果：选用F6双J管（甲组）术后两周腰痛8例、血尿59例、膀胱刺激症36例、尿路感染12例、双J管腔结石4例、术后第1周碎石排出10%、术后第2周碎石排出35%、术后第4周碎石排出75%。选用F4.5双J管（乙组）术后两周腰痛5例、血尿48例、膀胱刺激症28例、尿路感染11例、双J管腔结石2例、第1周碎石排出36%、术后第2周碎石排出50%、术后第4周碎石排出95%。两组患者术后并发症差异有显著统计学意义（P＜0.01），术后2周及4周碎石排除率差异具有统计学意义（P＜0.05）。结论：输尿管结石术中明显有碎石情况下选用小号双J管更有利于碎石排出。 The experience on the selection of the right types of double J tube performing the utter urinary tract stone operative lien. [Abstract] Objective: Ureteroscopy holmium laser surgery under different models gravel double double-J line- for the influence of the gravel discharge. Methods: 232 cases of patients under ureteral calculi urethral ureteroscopy holmium laser gravel was performed using transurethral ureteroscopy lien, different types of double J tube and observe the rubble after the row of stone. Results: use F6 double J tube (division 1) the two weeks of back pain 8 cases , hematuria 59 patients, bladder irritation disease 36 cases, urinary tract infection in 12 cases, double J lumen stone 4 cases ,the first week after gravel discharge, the second week 10% from 35%, the fourth week from 75%. Choose F4.5 double J tube (paper) the two weeks of back pain in 5 cases, hematuria 48 cases, bladder irritation 28 cases of disease, urinary tract infection in 11 cases, double J lumen calculus 2 cases, the first week from the rubble 36%, 2 weeks from the rubble 50%, 4 weeks gravel from 95%.The difference of the complication between the two groups has significant statistical meaning(P0.01),and the difference of the rubble rate between the first week and the second week has significant statistical meaning(P0.05).
