Inquiry General data: Shan Bao ,male ,44 years old ,farmer Chief complaint: discontinuous shortness of breath on exertion for 3 years ,recurred half month ago. History of present illness: Three years ago, the patient appeared short of breath on exertion and he was also suddenly awaked by the feeling of suffocation several times during sleep . The local hospital gave the diagnosis of “hypertrophic cardiomyopathy and atrial fibrillation” and treated him with cardiotonic and diuretic drugs which made him better. After discharge, he took captopril, spironolactone, and aspirin one pill a day, and started to take digoxin one year ago. 45 days ago, he found edema of his legs, so he took furosemide. A half month ago, he felt sensory loss of his leg for a few seconds and went to the local hospital. There’s no obvious effect after 2 days of treatment, so he turned to our hospital. The patient has no symptom of cough, no chest pain or palpitation, no dizzy or headache, no abdominal pain or distension. He has blurred vision occasionally. His diet and sleep is fine without weight loss. His stool and urine is normal. Past history: hypertension for 10 years, the highest BP reached 180/120mmHg; atrial fibrillation for 3 years. Denying diabetes, coronary heart disease or any contagious diseases, operations or trama history.
