JModUro1，Vo1．20No．1Jan．2015 · 临床研究 · 改良Clavien分级系统在经尿道前列腺等离子双极电切术围手术期并发症中的应用廖正明，肖和，王行环，方志海，吴文博，刘庆，杨中华 (武汉大学中南医院泌尿外科，湖北武汉 430071) Analysisofperioperativecomplicationsofplasmakineticbipolarresection ofthe prostate LIAO Zheng—ming，XIAO He，WANG Xing—huan，FANG Zhi-hai，WU W en—bo，LIU Qing， YANG Zhong—hua (DepartmentofUrology，ZhongnanHospitalofW uhanUniversity，W uhan430071，China) ABSTRACT：Objective Toretrospectivelyreviewandgradetheperioperativecomplicationsofplasmakineticbipolarresec- tionoftheprostate(PKRP)usingmodifiedClavienclassificationsystem (MCCS)．Methods Clinicaldataof171benignpros— tatehyperplasia (BPH)patientswhounderwentPKRPduringJan．andDec．2013wereretrospectivelyanalyzed．Accordingto prostatesize，patientswereclassifiedintothreegroups：prostate 40mL (groupA)，40～79mL (groupB)，and≥80mL (groupC)．ComplicationswererecordedusingMCCS．Results Thedifferencebetweenthethreegroupswasnotstatistically significantinage，maximum urinaryflow rate，postvoidresidualurine，internationalprostatesymptom scoreandqualityoflife (P0．05)．Duringhospitalization，43complicationsoccurredin34cases，withthetotalcomplicationratebeing19．9 (34／ 171)．Indetail，theincidenceofcomplicationswas11．5 (3／26)ingroupA，20．7 (3／26)ingroupBand22．4 (13／58)in groupC (Z 1．405，P0．05)．Amongthese43complications，22weregradeI，16weregrade Ⅱ，1wasgradeIlIa，3were grade1V，and1wasgradeⅣa．TherewerenogradeIlIbandV complications．Conclusion UsingthemodifiedClavienclassi- ficationsystem，gradesIandIIcomplicationsthatrequirenononinterventionaltreatmentconstitutethemajorityofcomplica- tions(88．4 )，whilegrades1II(2．3 )andVI(9．3 )complicationswerelesscommonandgradeV complicationswerezero (O％)．TheMCCSisavalidatedmethodthatcanbesuccessfullyusedtogradecomplicationsfollowingPKRP． KEY W oRDS：benignprostatehyperplasia；transurethralplasmakineticbipolarresectionoftheprostate；modifiedClavien classificationsystem 摘要：目的采用改良的Clavien分级系统对经尿
