新型农村合作医疗制度可持续发展的研究____基于政府责任的视角.doc

摘  要  伴随着我国农村经济体制改革的不断深入，20 世纪 60 年代发展起来的合作医 疗所依附制度环境的改变，使得这一体系也在逐步瓦解。虽然在上个世纪 90 年代， 国家提出了“恢复和重建”农村合作医疗制度，但并没有及时依据整个社会经济发展 的状况对旧制度下的合作医疗做出制度调整，因而造成重建合作医疗制度投入不 足，导致地方政府在制定政策时缺乏统一性和稳定性，最终影响重建工作的效率。 农民成为最大的自费医疗群体，因病致贫、返贫问题突出。为了解决农村基本卫 生医疗保障问题，2002 年 10 月在《关于进一步加强农村卫生工作的决定》中，中 共中央、国务院提出了在全国建立新型农村合作医疗制度，并于 2003 年在全国试 点运行。在政府的组织和政策的引导下，新型农村合作医疗制度在全国范围得到 极快的推广。新型农村合作医疗之所以能取得如此显著的绩效，关键原因是政府 的积极参与和承担了主要的责任。因此，要使新型农村合作医疗获得可持续发展， 必须有政府这股重要力量的积极参与。 本文首先回顾和总结了新型农村合作医疗的理论基础和国内外相关研究状况， 探讨了新型农村合作医疗制度的特点，通过阅读大量文献归纳总结出新型农村合 作医疗制度持续发展所需要的支撑条件，并重点分析了这些条件所存在的问题与 不足，不利于新型农村合作医疗制度的持续发展，接着从政治学、经济学、法学 和社会学等多学科角度详细阐述了政府在新型农村合作医疗制度中承担责任的必 然性，为政府责任的承担提供了必要的理论依据，在此基础上，提出完善这些条 件政府应当怎么做，即政府责任定位。 关键词：新型农村合作医疗制度，可持续发展，政府责任 I ABSTRACT With the reform Chinas rural economic system, dependent of peoples communes and the special medical system under the planned economy system,after all these systems’ environment disappeared, the rise and development of the cooperative medical treatment in1960, gradually collapsed. Since the 1990s, the relevant department of the central government proposed recovery and reconstruction rural cooperative medical system, but the country doesn’t timely adjust corresponding system of traditional cooperative medical, according to the rural actual economic and social development, makes the reconstruction of cooperative medical system lack of necessary legal basis and system insufficient input, leading to the government at all levels of cooperative medical policies formulated the height not stability and skill, causing reconstruction of low efficiency of work. So the phenomenon that farmers became impoverished by hospitalization or illness has expanded, the poverty problems became serious.
