健脾化痰复方改善非酒精性脂肪肝大鼠胰岛素敏感性的机制研究 王 磊 李 勇 胡冬青 孙欢娜 侯辰阳 高 波 山东中医药大学附属医院肝病科 （济南，250011 ） 【摘要】目的 观察健脾化痰复方对非酒精性脂肪肝 （nonalcoholic fatty liver disease， NAFLD ）胰岛素抵抗 （IR ）的治疗作用，并探讨其可能的作用机制。方法 采用高脂高热 量饮食饲喂 12 周制备NAFLD 大鼠模型，成模后以二甲双胍为阳性对照，灌胃健脾化痰复 方 6 周，18 周末处死全部动物，观察肝脏指数、肝脏组织病理、血清肝功能、血脂、肝脂 含量及胰岛素抵抗指数。结果 与NAFLD 大鼠比较，健脾化痰复方组大鼠肝脏指数、血清 ALT 、AST 活性、血清胆固醇、游离脂肪酸含量及肝组织甘油三酯、胆固醇和游离脂肪酸含 量以及胰岛素抵抗指数均显著降低 （P 均﹤0.01 ）；与二甲双胍组比较，健脾化痰复方组胰 岛素抵抗指数无显著差异（P ﹥0.05 ），但血清及肝组织游离脂肪酸含量均显著低于二甲双胍 组（P 均﹤0.01 ）。结论 健脾化痰复方改善NAFLD 大鼠胰岛素敏感性的作用与二甲双胍相 当，减轻高FFA 血症是其突出的作用特点。 【关键词】脂肪肝，非酒精性；健脾化痰复方；胰岛素抵抗 Study on therapeutic effect of Jianpi Huatan decoction on insulin sensitivity in rats with nonalcoholic fatty liver disease WANG Lei, LI Yong, HU Dong-qing, SUN Han-na, HOU Chen-yang, GAO Bo. Methods Forty-one male Wistar rats were randomly divided into Jianpi Huatan decoction-treating group, metformin -treating group, week 12 control group and week 18 control group (natural recovery group). NAFLD model was estabished by feeding with high-fat, high-calorie diet for 12 weeks . Liver index, liver histology, serum ALT, AST, TC, FFA, HOMA-IR and liver lipid contents were arrayed at the end of week 18. Results Compared with NAFLD control group, liver index, serum ALT, AST, TC, FFA, HOMA-IR and liver lipid contents decreased significantly (P ﹤0.01) in Jianpi Huatan decoction-treating group with significant improvement in hepatic steatosis.
