276 西北国防医学杂志 2011 年8 月30 日第32 卷第4 期 Med J NDFNC ，VOL． 32 ，NO． 4 ，August ，2011 AFP 、CEA 和CA199 联合检测在原发性肝癌诊断中的应用 董菊子，邓芝云，张 峰，彭俊华 (兰州军区兰州总医院检验科，甘肃兰州730050) ［摘要］ 目的:探讨多种血清肿瘤标志物联合检测对原发性肝癌的诊断价值。方法:采集110 例原发性肝癌和152 例良性肝 病(肝硬化和慢性肝炎)患者血液，用化学发光法测定AFP 、CEA 和 CA199 水平。结果:原发性肝癌患者血清中AFP 、CEA 和 CA199 水平分别为(60 ． 6 ± 80 ． 5)g / L 、(22 ． 2 ± 13． 9)g / L 、(47 ． 2 ± 28． 3)U / L ，明显高于良性肝病患者血清水平;原发性肝癌 μ μ 患者中AFP 、CEA 和CA199 阳性率分别为74 ． 6% 、49 ． 1% 和45 ． 5% ，显著高于良性肝病患者;三者联合检测将原发性肝癌检测 敏感率提升至92 ． 7% 。结论:联合检测可提高原发性肝癌诊断的敏感率，在原发性肝癌的筛查中有一定的应用价值。 ［关键词］原发性肝癌; AFP ; CEA ; CA199 ［中图分类号］R 735 ． 7 ［文献标识码］A ［文章编号］1007 － 8622 (2011)04 － 0276 － 02 Diagnostic value of combined detection of AFP ，CEA and CA199 in primary liver cancers DONG Ju － zi ，DENG Zhi － yun ，ZHANG Feng ，et al． (Department of Laboratory ，Lanzhou General Hospital ， Lanzhou Command ，PLA ，Lanzhou 730050 ，China ) ［Abstract］ Objective :To study the clinical value of combined detection of the serum levels of AFP ，CEA and CA199 in primary liver cancers． Methods :Collecting the serum samples from 110 primary hepatic cancer patients and 152 benign liver disease patients (hepatic cirrhosis and chronic hepatitis)，the serum levels of AFP ，CEA and CA199 were measured by chemiluminescence． Results :The levels of AFP ，CEA and CA199 were (60 ． 6 ± 80 ． 5) g / L ，(22 ． 2 ± 13． 9) g / L and (47 ． 2 ± 28． 3)U / L in primary liver cancer patients respectively ，it was signifi- μ μ cantly higher than that in benign liver disease patients． The serum positive ratio of AFP ，CEA and CA199 in prima- ry liver cancer patients were 74 ． 6% ，49 ． 1% and 45 ． 6% ，respectively ，it was significantly higher than that in benign liver disease patients too． The sensitivity of combined detection of three kinds of tumor markers was 92 ． 7 % ． Conclusion :The combined detection of AFP ，CEA and CA19
