ADM包扎治疗Ⅱ度烧烫伤创面临床效果的观察 【摘要】 目的 探讨ADM(异种脱细胞真皮基质)包扎治疗Ⅱ°烧烫伤创面的临床效果。 方法 随机采用异种脱细胞真皮基质包扎治疗（治疗组）和使用磺胺嘧啶银乳膏包扎治疗（对照组）的Ⅱ°烧烫伤创面各50例患者。比较两组在换药后疼痛、换药次数、预后瘢痕增生等方面的差异。 结果 治疗组0-Ⅰ级疼痛入院时22例（44%），治疗后48例（96%），差异具有统计学意义（p0.01）；对照组0-Ⅰ级疼痛入院时23例（46%），治疗后22例（44%），差异无统计学意义（p0.05）。两组治疗后0-Ⅰ级疼痛率比较，差异有明显统计学意义（p0.01）。治疗组患者创面愈合后比对照组皮肤颜色更浅、弹性更好。半年后创面疤痕形成有明显差异（p0.01）。结论：异种脱细胞真皮基质用于治疗Ⅱ°烧烫伤创面具有止痛快、促进创面愈合，减少瘢痕增生，减轻患者痛苦，减少医生劳动量等优点。ADM是一种较理想的内层包扎敷料。 【关键词】ADM包扎，Ⅱ度烧烫伤，临床效果 Clinical effect evaluation of ADM dressing on the wound healing of second degree burn Abstract Objective To investigate the clinical effect of ADM (Heterogeneous Acellular dermal matrix) on the treatment of burn wounds in the second degree burn. Methods 50 patients were randomly divided into treatment group (treatment group) and the treatment group (control group) with the use of the Heterogeneous Acellular dermal matrix (treatment group). The differences of the two groups in the change of the pain, the number of dressing change, and the prognosis of scar hyperplasia were compared. Results in the treatment of hospitalized with group 0 class I pain in 22 cases (44%), after treatment of 48 cases (96%), the difference is statistically significant (P 0.01); group 0 class I pain on admission control 23 cases (46%), after treatment in 22 cases (44%), the difference was not statistically significant (P 0.05). In the two groups after treatment, there was significant difference in 0- grade pain rate (p0.01). The patients in the treatment group were lighter and more flexible than the control group. After six months, there were significant differences in wound scar formation (p0.01). Conclusion: xenogenic deproteinization acellular dermal matrix for treating second degree burn wound has to relieve pain quickly, promote wound healing, reduce scar hyperplasia, to alleviate the suffering of patients, reduce doctor amount of labor and other advantages.
