对通辽市剖宫产率持续升高问题调查研究.pdf

摘 要 目的：通过调查通辽市地区剖宫产指征变化的情况，分析通辽市地区剖宫产率 持续升高的原因，讨论如何降低剖宫产率。方法：抽查多家医院在2006年1 月一2010年12月期间的14961例住院分娩产妇资料进行回顾性分析。其中14961 例为做调查剖宫产率情况，600例病例做为调查剖宫产指征。结果：①剖宫产 率明显提高：2006—2010年剖宫产率分别为42．11％，48．92％．54．54％、60．97 ％、70．9％，2006年与2010年比较，差异有统计学意义(PO．01)；这5年的 剖宫产率呈逐年上升的趋势。②主要剖宫产指征的构成比变化：社会因素、头 篙不称、巨大儿、胎儿宫内窘迫、羊水过少、在各年剖宫产指征所占比例逐年 上升趋势。胎位异常、产程异常病例呈逐年下降趋势。社会因素变化最为显著， 从2006年的21％10 2010年27％，并差异有统计学意义(PO．01)。结论：1通 辽地区剖宫产率逐年上升。2剖宫产率升高受多种因素影响。3控制剖宫产率还 需要做大量的工作，需要社会各界共同努力。 关键词：剖宫产率；剖宫产指征 Discuss the high rate of cesarean section to tongliao Abstract objective：To observe indications of cesarean section，find a solution to decrease the high rate of cesarean section and ana．1yze the causes of cesarean section．in tongliao Methods：The indications of cesarean section in 2006—20 1 0year 1 496 1 cases wereanalyzed retrospectively．Among 1 43 6 1 cases to The rate of cesarean section，600 cases for indications of cesarean section Results：(1)since2006·2010 year rate of cesarean section：42．1 1％．48．92％．54．54％、60．97％、70．9％，The average rate of cesarean section was 42．1 1％from2006，it went up year by year from 2006and kept the high rate from 20 1 0．The rate of cesarean section was 70．9％in 20 1 0 and 42．11％in 2006，the difference between them was significant(P茎O．01)．(2) Among the indications of cesarean section from 20 1 0，the first was society factor(27 ％1．The others in turns was fetal distress，fetal position abnormal，breech presentation，oligohy2dramnios，cephalopelvic disproportion，etc．Society factor of cesarean section had raised from 21％in 2006 t027％in 201 0．the differ2ence of them was significant f P SO．01)．Conclusions：The high rate of cesarean section is it went up year by year．A lot of factor‘lead to The high of cesarean section．So find a solution to decrease the high rate of cesarean section must a lot of work． Key words：Cesarean section j Indications Di rected by：prof．BAO Bao Zhu App I i cant for Master degree：WU Tu Ya(clink ofChinese and Westem Integrative) (mongoliar．medicine colleage．Inner Mongolia
