肝素治疗肺心病呼吸衰竭80例临床观察 　　【摘要】 目的：探讨肝素治疗肺心病呼吸衰竭的效果及安全性。方法：将160例肺心病呼吸衰竭患者随机分为治疗组与对照组，各80例，两组均选择有效抗生素，同时给予平喘、祛痰、吸氧、强心、利尿、纠正酸碱中毒和电解质紊乱、治疗各种并发症等综合治疗。治疗组在此基础上应用肝素50～100 mg，加入到250 ml 5%葡萄糖注射液中静滴，1次/d，7～10 d为一疗程。比较两组患者的治疗效果，以及治疗前后凝血时间、血小板计数及临床出血倾向。结果：治疗后，治疗组显效率和总有效率均明显优于对照组，两组比较差异均有统计学意义（P0.05）。两组患者治疗后血小板计数、出血和凝血时间均无明显变化，临床无1例发生出血倾向。结论：肝素治疗肺心病呼吸衰竭效果显著、安全，无毒副作用，是治疗肺心病呼吸衰竭的一种有效的方法。 　　【关键词】 肺心病； 呼吸衰竭； 肝素； 出凝血时间； 疗效 　　中图分类号 R563.8 文献标识码 B 文章编号 1674-6805（2016）10-0097-02 　　【Abstract】 Objective：To investigate the efficacy and safety of heparin in the treatment of pulmonary heart disease respiratory failure.Method：160 patients with pulmonary heart disease respiratory failure were randomly divided into the treatment group and the control group，80 cases in each group.Two groups both selected effective antibiotics，the antiasthmatic，expectorant，oxygen，strong heart，diuresis，corrected poisoning of acid-base and electrolyte disturbance，treatment of complications such as comprehensive treatment were given at the same time.On the basis of the treatment above，heparin 50-100 mg was added to the 5% GS 250 ml in the intravenous drip，one time a day，7-10 days for a course of treatment.Observed the effect of the two groups，the blood coagulation time，platelet count and clinical observation of bleeding tendency.Result：The efficiency and the total effective rate in treatment group were better than the control group，the differences were statistical significance（P0.05）.There were no significant changes in platelet count and blood coagulation time after treatment，and there was no 1 case had clinical bleeding tendency.Conclusion：Heparin is effective，safe and non-toxic side effects in treatment of respiratory failure of chronic pulmonary heart disease，it is an effective method for treating pulmonary heart disease with respiratory failure. 　　【Key words】 Pulmonary heart disease； Respiratory failure； Heparin； Clotting time； Curative effect 　　First-author’s address：Cao County Traditional Chinese Medicin
