探讨大枫子皮肤舒对牛皮癣性关节炎临床影响 寻找牛皮癣性关节炎的x线征象特点作鉴别诊断。方法选择经临床确诊的具有典型牛皮癣皮肤病变的牛皮癣性关节炎患者，拍摄足部正、侧、斜位平片，分析牛皮癣性关节炎的X线表现及影像特点。结果通过x线影像分析，牛皮癣性关节炎的突出表现在掌指和跖趾关节的病变。x线表现为远端掌指关节或跖趾关节间隙狭窄．关节面可有虫蚀样骨质破坏缺损，末节指或趾骨近端有不规则骨增生并呈杯状膨大，末端骨质可吸收变尖变细。甚至消失，重者关节强直，病理性脱位，屈曲畸形。结论牛皮癣性关节炎临床发现用大枫子皮肤舒结合临床实验。对牛皮癣性关节炎有一定效果。 X—ray Analysis of Misdiagnosis in 35 Cases of Psoriatic ArthritisYANG Yeyou(Department ofRadiology,Ruili Second People 3 Hospital,Ruli 678600,China)【Abstract] Objective To improve the recognition ofpsoriatic arthritis,search the X-ray signs characteristics ofpsoriatic arthritis foriagnosis．Methods Confirmed psoriatic arthritis patients with typical psoriasis dermatopathy were choosed to take pictures of feet frontal，lateral and oblique plain films；analysed the X-ray performance and image characteristic of psoriatic arthritis．Results The distinctmanifestations ofpsoriatic arthritis were the lesion in metacarpophalangeal and metatarsophalangeal joints．The performance ofX-my werethe stenosis of distal metacarpophalangeal joints or metatarsophalangeal joints，the mouse nibbled bone destruction in articular surface，irregular bone proliferation with cotyliform intumescence in distal seg mentof finger and proximal phalanx，the end absorbtion became sharpand attenuation even disappeared，arthrocleisis，pathological dislocation and flexiondeformity happened in severe patients．nclusionPsoriatic arthritis has low incidence in clinic，and is easy to be ignored，misdiagnosed and mistreated，causing fuller deterioration．It’S notdifficult to diagnose the psoriatic arthritis through the X-my image analysis to search the differentia between Other kinds ofarthritis．【Key words】Psoriatic arthritis；Misdiagnosis；X-ray；Analysis 牛皮廨性关节炎在牛皮廨患者中发病率不高．约3—5％．临床上往往易被忽视，特别在类风湿性关节炎发病率高的情形下．常常易把牛皮廨性关节炎误诊为类风湿性关节炎而耽误护理，造成关节病变进一步恶化。笔者自1989至2010年的临床工作中，收集了35例被误诊的牛皮廨性关节炎病例运用大枫子皮肤舒局部护理进行分析和探讨。一般资料35例误诊的牛皮廨性关节炎，男性24例．女性11例，男女比例约为2：1。年龄：15—48岁。病程最短2年，最长7年。23例误诊为类风湿性关节炎，12例误诊为风湿性关节炎。关节炎发生在掌指关节、指间关节者15例，占42．9％，其中掌指关节、指间关节严重变形并关节脱位、功能丧失者9例。发
