申请设立民办学校流程图 ? ? （一）申办条件 1、办学规模不少于200人； 2、场地总面积不少于400平方米，租赁期不少于3年； 3、固定资产不少于20万元（如单一电脑培训，要求配备电脑120台）； 4、配备校长1名，持高级以上职业资格证书；每个培训工种配备教师2名，持中级以上职业资格证书（如单一电脑培训，应配备教师4名）；财务1名，持会计证； 5、《民办职业培训学校设置标准（试行）》规定的其他条件。 （二）所需材料 1、申办报告。内容包括：学校名称及地址、举办者姓名及联系电话、办学规模、办学条件（必须注明场地、设备、资产、师资等情况）、办学形式、培养目标、培训专业（工种）及层次、内部管理体制、经费筹措与管理使用等。 2、申请表一式两份。 3、法定代表人身份证复印件两份。 4、校长、教师、财会人员的资格证原件及身份证、学历证复印件、聘用合同。 5、办学场所租赁合同（租赁期不少于3年）、产权证明（有房产证，提供复印件；如无房产证，提供村委会出具的房产产权证明）。 6、学校资产证明（提供会计师事务所出具的验资报告，数额20万元以上）。 7、学校章程（按范本格式写）。 8、各项管理制度。如教学管理、教师管理、学生管理、财务管理、卫生安全管理等制度。 9、招生简章、广告。要如实说明教学目标和承诺、设备设施、教学实习条件、开设课程、培训专业（工种）、时间、总课时数、各项收费、考试发证等。 10、教学计划、大纲（分培训工种编写）。 11、提供教学场地门面、前台、教室、设备相片一套。 （三）审批部门及咨询电话 审批部门：东莞市劳动局职业技能培训科 地址：东莞市花园新村育兴路101号 电话 to East for the neijiang-Kunming highway bridge across the River, maming Creek Bridge, pond bridge, bridge, rongzhou bridge, and South Bridge.
