脑卒中后病人实施人文关怀护理的效果 上海市中医医院200071鲁剑萍 张洁 【摘要】 目的 探讨心理、人文关怀对脑卒中后轻度抑郁患者康复的影响。方法 60例首发脑卒中后抑郁患者，随机分成两组各30例，对照组给予常规护理及抗抑郁治疗，研究组在常规护理的基础上，结合人文关怀模式进行护理，并在干预前与干预1月、2个月通过HAMD量表评分、住院病人满意度问卷调查，评价人文关怀模式对脑卒中抑郁患者的效果。结果 治疗2个月后，两组患者HAMD评分与治疗前比较均明显降低(P0.05),住院病人满意度调查较对照组有所提升。两组比较,研究组各项评分下降更显著(P0.05)。结论 脑卒中后轻度抑郁症患者能接受心理人文关怀,同时心理人文关怀可以显著改善患者的抑郁程度。 【关键词】脑卒中后抑郁 人文关怀 心理 【Abstract】Objective: To investigate the effects of psychological intervention and human care on depression and function rehabilitation following depression. Methods: 60 patients with post stroke depression were divided into treatment group and control group randomly with each 30 patients. The patients in the control group were accepted routine neurological therapy. The patients in the treatment group were additionally treated with psychological intervention and human care. Then give the surveys by HAMD scale score and patient satisfaction before the intervention and intervention in one and two months, evaluation of humane care model the effect of depression on stroke. Results: After treatment for 2 months the scores of HAMD were significantly reduced as compared with those before treatment (P0.05). Patient satisfaction survey has improved compared with the control group.The reduction of the scores of HAMD was more significant in treatment group than in control group (P0.05). Conclusion: The patients with post stroke depression can accept psychological intervention and human care. And psychological intervention and human care may obviously improve post stroke depression and limb function. 【Key words】post stroke depression; psychological intervention; human care 脑卒中后抑郁症(Post stroke depression, PSD)是指脑卒中后发生的以情绪低落、兴趣减退为主要临床表现的病症,是脑血管病常见并发症之一,不仅导致躯体症状的加重,而且加重病人的精神痛苦,使其生存质量降低[1],也妨碍神经功能的恢复,同时给家庭和社会带来沉重的经济负担和压力。而人文关怀护理是以尊重病人的生命价值、人格及个人隐私为核心,给予病人心理干预与人文关怀护理,是一种创造性的、个性的、整体性的、有效的护理模式。我科自2010年以来, 针对不同病人的生理、心理、社会需求提供人文关怀护理,提高病人的舒适度及生活自理能力,使他们最大限度地回归社会具有重要的意义。现介绍如下。 1　资料与方法 1.1一般资料　2010年1月至2011年1月在上海市中医医院住院的脑卒中伴抑郁患者60例。诊断标准: (1)首次发
