肾上腺素盐水纱布压迫防治产后出血的临床应用 　　doi：10.3969/j.issn.1007-614x.2014.12.41 　　摘 要 目的：对肾上腺素盐水纱布压迫治疗产后出血的临床应用进行分析总结。方法：对16例产妇在胎儿娩出后，发现胎盘剥离面渗血，助手立即挤压子宫，术者迅速将肾上腺素（1mg）盐水纱布压迫于渗血处，5分钟后，边压迫边缝合子宫切口，关闭子宫切口前取出纱布，观察出血、血压情况。若是子宫下段、宫颈内膜渗血，则将肾上腺素（1mg）盐水纱布压迫于子宫下段，并将少许纱布头引出宫颈口（便于术后经阴道取出），边压迫边缝合子宫切口，观察出血、血压情况。结果：用手法压迫+子宫肾上腺素盐水纱布压迫治疗产后出血共16例，治疗效果显著，治愈率100%。无一例切除子宫，无再出血现象。结论：手法压迫和肾上腺素盐水纱布压迫治疗产后出血能明显降低产后出血量，操作方法简单、迅速、有效、安全，临床应用效果好，值得推广。 　　关键词 肾上腺素 防治 产后出血 　　Clinical application of adrenaline saline gauze compression in the prevention of postpartum hemorrhage 　　Yang Danjie，Chen Xiufang 　　Department of Gynaecology and Obstetrics，the Fifth Peoples Hospital of Fuyang City，Anhui 236027 　　Abstract Objective：To analyze and summarize the clinical application of adrenaline saline gauze compression in the prevention of postpartum hemorrhage.Methods：After the fetus childbirth，the placental separation surface of 16 cases of maternal were found bleeding.The assistant immediately squeezed the uterus，then the surgeon quickly oppressed epinephrine（1mg） saline gauze in place oozing.Five minutes later，the surgeon oppressed gauze，at the same time stitched uterine incision，and remove the gauze before closing the uterine incision.The bleeding situation was observed，while the blood pressure was observed whether was stable.If the lower uterine segment and endocervical were found bleeding，the epinephrine（1mg） saline gauze was oppressed in the lower uterine segment，and a little gauze head was leaded to the cervix（be easy to remove after vaginal），the surgeon oppressed gauze，at the same time stitched uterine incision.The bleeding and blood pressure situation were observed.Results：16 cases of postpartum hemorrhage were treated with manual compression and adrenaline saline gauze compression，the treatment effect was significant.The cure rate was 100%.No case had hysterectomy and bleeding again.Conclusion：The manual compression and adrenaline saline gauze compression in the treatment of postpartum hemorrhage can sign
