《心脏术后合并真菌感染51例临床分析.docVIP

【摘要】 　　目的：分析和总结体外循环心脏手术后合并真菌感染的临床特点及耐药情况。方法：对51 例体外循环心脏直视术后继发真菌感染的临床资料进行统计分析，分析术后培养分离出的78 株感染真菌的药敏试验结果。结果：心脏术后长期使用广谱抗生素及皮质激素、机械通气、气管切开、A P A C H E Ⅱ 高评分、全肠外营养及侵入性检查治疗等均是真菌感染高危因素。心脏术后真菌感染部位以下呼吸道为主，其次为血液、静脉导管顶端，继之为泌尿系。大多数真菌菌株对两性霉素B 、5 -氟胞嘧啶敏感，对大扶康及伊曲康唑部分耐药。结论：合理使用广谱抗生素和糖皮质激素，尽量减少不必要的侵入性操作，加强全身营养及脏器功能支持，改善肠道微生态平衡，密切监测真菌感染的一切临床迹象，注重预防及经验性治疗；治疗上应选择敏感、毒副作用轻的抗真菌药物。 【关键词】 心脏手术；感染；真菌 　　Clinical analysis of 51 cases of fungal infection post cardiac surgery 　　X U W eijiang ，LIU Bin ，LIA N G Yan2e 　　ChineseJournal of Cardiovascular Rehabilitation M edicine ，2007 ，16 （4 ）：377 Abstract：Objective ：To explore the clinical characteristic and drug resistance of true fungal infection in open cardiacsurgery .Methods：Statistical analysis of 51 cases of true fungal infection post cardiac surgery ，and the analysis of drug susceptibility of 78 cases positive cultures for fungalinfection were performed .Results ：The risk factors of true fungalinfection were the use of extended2spectru m anti2microbial agents and corticosteroid therapy 、mechanical venti2 lation 、high A P A C H E Ⅱ grade 、total parenteral nutrition 、invasived operation and treatment .The most co m mon sites of infection were respiratory tract ，followed by blood ，top of venous duct，urinary system .M ost true fungalpathogens were sensitive to am photericin B 、5 -Flurocytosine and co m mon antibiotic to fluconazol、itraconazole .Conclusion ：The proper use of anti2microbial agent and corticoteroid ，grasp the indication of intervened diagnosis andtherapystrictly ，nutrition and organ support should be supervised .Its risk factors should be avoided . Key words ：Cardiac surgery ；Infection ；True fungus 　　随着体外循环技术及心脏外科手术的不断进步以及心脏移植的不断开展，使得大量心功能衰竭及终末期心脏病患者得到手术救治。术后广谱抗生素、免疫抑制剂和糖皮质激素的临床广泛使用，以及更多的有创监测与治疗的临床应用，使得心脏术后真菌感染的发病率与死亡率日益增多。为了解心脏术后真菌感染临床特点，本文就2006 年1 月， 2006年12 月发生的51 例心脏术后真菌感染患者的临床资料进行回顾性分析。现报道如下。 1 资料与方法 1.1 一般资料 51 例真菌感染病例中，男33 例，女18 例，年龄29 ， 79 ，平均（61136 ±16127 ）岁。真菌感染患者原发病及手术情况：冠心病搭桥手术
