Shackle not fully screwed in No mousing Lifting point SWL? Condition of the ‘fall’ rope Single sheave, wooden block used in an ‘escape’ shaft Should only be used for lifting personnel in an emergency, not general purpose lifts (stores etc!) Down a dead-end Touching the deck in a dirty area Pedestal drill (Hot Swarf) Rag bin The EEBD should be stowed near the exit, away from machinery (hot drill swarf) the oily rag bin Extra EEBDs are provided for drills and training – use them! Better stowage position – near the emergency exit Retro fit security measures *
