学术成果报告雷琳2013-05-16视黄醇结合蛋白4(RBP-4)与糖尿病肾病关系的研究Study on the relationship between RBP-4and diabetic nephropathy摘要:目的:本实验将探讨2 型糖尿病患者血清视黄醇结合蛋白4 ( serum retinol binding protein 4, sRBP-4)与糖尿病早期肾病的关系。实验方法:2 型糖尿病(diabetes mellitus, DM) 患者100 例,根据24 h 尿白蛋白排泄率(UAER)分为正常蛋白尿组(UAER ≤20 ug/min),35例;微量蛋白尿组(20ug/min UAER 200 ug/min),34例;大量蛋白尿组(UAER ≥200 ug /min),31例。正常对照组(NC 组) 35 例,检测血清RBP-4含量。结果:血清RBP-4含量在2型糖尿病患者正常尿蛋白组(50.26 ± 6.29 mg/L)、微量尿蛋白组(66.39 ± 12.81mg/L)和大量尿蛋白组(76.16± 19.21mg/L)均高于正常对照组(27.75 ± 2.89 mg/L),而且在微量尿蛋白组(66.39 ± 12.81mg/L)和大量尿蛋白组(76.16 ± 19.21mg/L) 血清RBP-4含量亦明显高于糖尿病正常尿蛋白组(50.26 ± 6.29 mg/L)。糖尿病患者的血清RBP-4含量与血液尿素氮、尿酸和肌酐含量成正相关。糖尿病患者的血清中RBP-4水平与GFR水平呈负相关。结论:实验结果显示血清RBP-4 是早期诊断糖尿病肾病的有利方法,这个指标可以作为早期发现糖尿病患者肾损伤的标记物。Abstract:Objectives: The purpose of this paper is to appraise the relevance of serum retinol-binding protein 4 (sRBP-4) with early diabetic nephropathy in type 2 diabetic patients.Methods: 100 type 2 diabetic patients were classified into three groups, normal-albuminuria subgroup, microalbuminuria subgroup and macroab- inuria subgroup, according to their urinary albumin excretion rate during 24 hours (UAER), and compared with the normal control group (group C) including 35 healthy people. Serum rentinol binding protein 4 (RBP-4) were measured in the 100 diabetic patients and 35 healthy persons.Results: Serum RBP-4 was significantly elevated in type 2 diabetic patients with normoalbuminuria (50.26 ± 6.29mg/L), microalbuminuria (66.39 ± 12.81 mg/L) and macroalbuminuria (76.16 ± 19.21mg/L) as compared with control patients (27.75 ± 2.89mg/L). Serum RBP-4 was also significantly elevated in type 2 diabetic patients with microalbuminuria or macroalbuminuria as compared with the normoalbuminuric group. Serum RBP-4 in diabetic subjects was positively correlated with BUN, uric acid and creatinine. Multiple stepwise linear regression analysis showed that GFR were significantly associated with serum RBP-4. Conclusions: Our results suggest that measurement of the serum
