[Method] Undergraduate in May 1985 ~ December 2003 OK hip arthroplasty in 119 patients (138 hips) were followed up in patients with femoral head necrosis. The use of artificial bipolar femoral head replacement and THA in treatment of advanced necrosis of femoral head were 29 patients (31 hips), 90 patients (107 hips). All operations are used after the lateral approach. [Results] Early use of bone cement made of artificial bipolar femoral head prosthesis, 867% of the femoral prosthesis stem loosening. All cemented THA, 50% appeared in acetabular cup hip prosthesis loosening, 625% hip prosthesis femoral stem loosening, 25% of the hip line of total hip arthroplasty. Hybrid (Hybrid) THA, does not appear acetabular cup and femoral stem prosthesis loosening. Non-cemented THA, except an exception without acetabular cup and femoral stem prosthesis loosening or subsidence. [Conclusion] The use of artificial bipolar femoral head replacement for treatment of advanced necrosis of the femoral head should be carefully chosen, suitable for the elderly, patients with small amount of daily activities; while for younger patients, non-bone cement-based THA as the best choice; for the femoral side appears osteoporosis or not suitable for the use of bio-fixed femoral stem prosthesis in patients with mixed-type (Hybrid) THA have equal access to satisfactory outcome; full Cementless THA should be rarely used.
