Methods: A retrospective analysis of our hospital from 2003 to 2007 50 cases of outpatient prescription errors deployment situations and consequences. Results: The outpatient prescription errors due mainly to three major types: non-standard prescription writing errors led to 14 cases of transfers, accounting for 28%; due to drug names, packaging and similar errors caused 19 cases, accounting for 38%; pharmacists not strict examination result errors in 17 cases, accounting for 34%. Conclusion: The deployment to eliminate outpatient prescription errors in a long-term one and should step up its vigilance. Methods: A retrospective analyze was made on the 50 cases of mistaken prescription distribution in the out  patient department during the years of 2003 ~ 2007. Results: There were three major causes leading to the faults: among all the 50 cases, 14 cases (28%) resulted from the physician  s nonstandard handwriting; 19cases (38%) from miscarrying out the dispensing regulation; 17 cases (34%) from negligence in examing of the prescriptions. Conclusions: It requires long  term effort to get rid of the errors in prescription distribution in the out  patient clinic and so an awareness against the error should be stressed on among the pharmacists.
