Hummel Early Postoperative Therapy after Heart Transplantation Early Postoperative Therapy after Heart Transplantation: Prophylaxis, Diagnosis and Antibiotic, Antimycotic and Antiviral Therapy of Infections Manfred Hummel Paulinenkrankenhaus, Berlin, Germany Aside from early graft failure,infections are the second leading cause of death after heart transplan- tation (HTx).Therefore, prophylaxis, diagnosis and adequate therapy of infections are fundamental for the short and long-term success of heart transplantation. Because immunosuppressive drug therapy is a prerequisite for transplantation, the range of infection is much larger than after other kinds of major thoracic operation.For prophylaxis,diagnosis and therapy,bacterial, viral,fungal and protozoal infections have to be taken into account, also as part of opportunistic infection.So far ev- idence-based recommendations for the prophylaxis and therapy of infections after HTx are rare. Therefore, it is necessary to use clinically proven strategies to prevent and treat infections. Key words: heart transplantation, infection, immunosuppression Frühe postoperative Therapie nach Herztransplantation: Prophylaxe, Diagnose und antibiotische, antimykotische und antivirale Therapie von Infektionen Neben dem frühen Transplantatversagen stellen Infektionen die zweith?ufigste Todesursache nach Herztransplantation (HTx) dar.Die Prophylaxe,Diagnose und richtige Therapie von Infektionen sind daher von grundlegender Bedeutung für den kurz- und langfristigen Erfolg einer Herztransplanta- tion. Die immunsuppressive Behandlung ist eine Grundvoraussetzung für die Transplantation. Da- her muss mit einer deutlich h?heren Rate an Infektionen gerechnet werden, als bei vergleichbaren gro?en thorakalen Operationen. Bei der Prophylaxe, Diagnose und Therapie müssen neben ambu- lant oder nosokomial erworbenen bakteriellen, viralen Pilz- und Protozoeninfektionen auch oppor-
