ADA、LDH、CEA联合检测在肺癌合并胸水中的诊断价值 中文摘要 目的 通过分别测定患者血清和胸水中腺苷脱氨酶（ADA ），乳酸脱氢酶 （LDH ），癌胚抗原（CEA ）的水平，探讨血清和胸水ADA 、LDH 、CEA联合检测 对肺癌合并胸腔积液的诊断价值。 方法 收集2012年12月-2013年6月在蚌埠医学院第一附属医院因胸腔积液就 诊、住院的患者100例为研究对象分为结核组45例，恶性组55，均行常规抽取静脉 血及常规胸腔穿刺抽取胸腔积液分别测定ADA 、LDH 、CEA 。用统计软件SPSS17.0 进行数据分析，P0.05表示差异有统计学意义。 结果 肺癌组血清和胸水中ADA含量明显低于结核组，其胸水与血清中ADA 的比值 也明显低于结核组（P0.05）。胸水及血清中LDH含量在结核组、肺癌组患者中比 较无显著性差异，但二者胸水中LDH含量均明显高于正常范围。肺癌组血清和胸水 中CEA 含量明显高于结核组，其胸水与血清中CEA 的比值也明显高于结核组 （P0.05）。联合检测ADA 、LDH和CEA对肺癌诊断的灵敏度为92%，特异度为78%， 准确度为86%，均明显高于各项标志物单独检测。 结论：肺癌组血清和胸水中ADA含量明显低于结核组；而肺癌组血清和胸水 中CEA含量明显高于结核组；联合检测血清和胸水中ADA 、LDH 、CEA对临床中 肺癌合并胸腔积液的诊断具有重要价值。 【关键词】ADA；LDH；CEA；肺癌；胸腔积液；联合检测 1 The diagnostic value study of ADA、LDH、CEA combined detection of pleural effusion of lung cancer ABSTRACT Objective Serum adenosine deaminase and pleural fluid adenosine deaminase (ADA), lactate dehydrogenase (LDH), carcinoembryonic antigen (CEA) levels were measured respectively, and the diagnostic value serum and hydrothorax ADA, LDH, CEA combined detection were evaluated in pleural effusion of lung cancer.
