[M e thod] wenty-four thoracolumbar m alignant neoplasm s patients treated by anterior approach total spondylectomy, spinal reconstractin w ith titanium m esh cage a-s sociated w ith bone graft or bone cement and posterior approach transpediuclar screw fixation. [R esu lt] he follow-up of postop- erationw as from 9 to 35 m onths.A llpatients achieved relieve of pain after surgery, 9 casesw ith incom plete paraplegia improved by 1.8 grades in average w ith Frankel neu rological classifications, 2 casesw ith dysfunction of urination and defecationw ere r-e covered, 4 patients decease formetastasis in critical organ, all the patients showed bony fusion at 6 ～9 m onths, no internal fix- ation loosening or breakage casew as found, 1 case was recrudescent after one year.
