Sorafenibisanoralangiogeneticmultikinaseinhibitorapprovedinthetreatmentofrenalandhepatocellularcarcinoma.Bleeding andvenousthromboticeventshavebeendescribedwithangiogeneticagentsbutcerebrovascularaccidentsarerarelyreported.We reporttwocasesofpatientswithhepatocellularcarcinomawhodevelopedacerebrovascularaccidentwhileonsorafenib.Neither patienthadanyriskfactorsforthecerebrovasculareventsapartfromgenderandageinthesecondpatient.Laboratorydatawere noncontributory.TheheadCTscandidnotrevealacuteabnormalities.Nohemodynamicallysigni?cantstenosiswasvisiblein thecarotidultrasound,andtheechocardiogramshowednormalsizeoftheheartchambersandnormalsystolicfunctionofthe leftventricle.Sorafenibwasdiscontinuedinbothcases.Physiciansshouldmonitorpatientsreceivingsorafenibforneurologic symptoms,andintheabsenceofotheretiology,promptdiscontinuationofthisdrugshouldbeconsidered. 1.Background (P = .05).
