Methods:Prospective,longitudinal,multicentretrialinthreecombinedmedicalandsurgicalintensivecareunitsof oneuniversityandtwogeneralhospitalsinSweden.Bymailedquestionnaires,healthrelatedqualityoflifewas assessedat6,12,24and36monthsafterthestayinICUbyEQ-5DandSF-36,andinformationofpre-existing diseasewascollectedatthe6monthsmeasure.ICUrelatedfactorswereobtainedfromthelocalICUdatabase. Comorbidityandhealthrelatedqualityoflife(EQ-5D;SF-36)wasexaminedinthereferencegroup.Amongthe 5306patientsadmitted,1663wereconsideredeligible(24hrsintheintensivecareunit,andage≥18yrs,and alive6monthsafterdischarge).Atthe6monthmeasure980(59%)patientsansweredthequestionnaire.Ofthese 739(75%)alsoansweredat12month,595(61%)at24month,and478(47%)answeredatthe36monthmeasure. Asreferencegroup,arandomsample(n=6093)ofpeoplefromtheuptakeareaofthehospitalswereusedin whichconcurrentdiseasewasassessedandadjustedfor. Results:Onlysmallimprovementswererecordedinhealthrelatedqualityoflifeupto36monthsafterICU admission.ThemajorityofthereductioninhealthrelatedqualityoflifeaftercareintheICUwasrelatedtothe healthrelatedqualityoflifeeffectsofpre-existingdiseases.Nosignificanteffectonthelong-termhealthrelated qualityoflifebyanyoftheICU-relatedfactorswasdiscernible. Conclusions:AlargeproportionofthereductioninthehealthrelatedqualityoflifeafterbeingintheICUis attributabletopre-existing
