Methods:Fourhundredninety-onefecalsampleswerecollectedfrom138patientswhowereadmittedtothe DepartmentofTraumatologyandAcuteCriticalMedicine,OsakaUniversityGraduateSchoolofMedicine,Japan. Thesepatientsweretreatedintheintensivecareunitformorethan2days.FecalpH,fecalorganicacids,andfecal bacteriacountsweremeasuredandcomparedbysurvivedgroupandnonsurvivedgroup,ornonbacteremiagroup andbacteremiagroup.LogisticregressionwasusedtoestimaterelationsbetweenfecalpH,age,sex,orAPACHEII scoreandmortality,andincidenceofbacteremia.Differencesinfecalorganicacidsorfecalbacteriacountsamong acidic,neutral,andalkalinefeceswereanalyzed. Results:TheincreaseoffecalpH6.6wassignificantlyassociatedwiththeincreasedmortality(oddsratio,2.46;95% confidenceinterval,1.25to4.82)orincidenceofbacteremia(3.25;1.67to6.30).Totalorganicacidwasincreasedin acidicfecesanddecreasedinalkalinefeces.Lacticacid,succinicacid,andformicacidwerethemaincontributors toacidityinacidicfeces.Inalkalinefeces,aceticacidwassignificantlydecreased.Propionicacidwasmarkedly decreasedinbothacidicandalkalinefecescomparedwithneutralfeces.Nodifferenceswerenotedamongthe groupsinbacterialcounts.
