Abstract
Introduction:Amongthevariousmethodsforimprovingoxygenationwhiledecreasingtheriskofventilation-
inducedlunginjuryinpatientswithacuterespiratorydistresssyndrome(ARDS),aventilationstrategycombining
proneposition(PP)andrecruitmentmanoeuvres(RMs)canbepracticed.Westudiedtheeffectsonoxygenationof
bothRMandPPappliedinearlyARDSpatients.
Methods:Weconductedaprospectivestudy.SixteenconsecutivepatientswithearlyARDSfulfillingourcriteria
(ratioofarterialoxygenpartialpressuretofractionofinspiredoxygen(PaO2/FiO2)98.3±28mmHg;positiveend
expiratorypressure,10.7±2.8cmH2O)wereanalysed.Eachpatientwasventilatedinboththesupineposition(SP)
andthePP(sixhoursineachposition).A45cmH2Oextendedsighinpressurecontrolmodewasperformedat
thebeginningofSP(RM1),onehourafterturningtothePP(RM2)andattheendofthesix-hourPPperiod(RM3).
Results:Themeanarterialoxygenpartialpressure(PaO2)changesafterRM1,RM2andRM3were9.6%,15%and
19%,respectively.ThePaO2improvementafterasingleRMwassignificantafterRM3only(P0.05).Improvements
inPaO2levelandPaO2/FiO2ratioweretransientinSPbutdurableduringPP.PaO2/FiO2ratiopeakedat218mmHg
afterRM3.PaO2/FiO2changesweresignificantonlyafterRM3andinthepulmonaryARDSgroup(P=0.008).This
globalstrategyhadabenefitwithregardtooxygenation:PaO2/FiO2ratioincreasedfrom98.3mmHgto165.6
mmHg13hourslaterattheendofthestudy(P0.05).PlateauairwaypressuresdecreasedaftereachRMand
overtheentirePPperiodandsignificantlyafterRM3(P=0.02).Somereversiblesideeffectssuchassignificant
bloodarterialpressurevariationswerefoundwhenextendedsighswereperformed.
Conclusions:Inourstudy,interventionssuchasa45cmH2OextendedsighduringPPresultedinmarked
oxygenationimprovement.CombinedRMandPPledtothehighestincreaseinPaO2/FiO2ratiowithoutmajor
clinicalsideeffects.
