Yeetal.DiabetologyMetabolicSyndrome2011,3:24 /content/3/1/24 DIABETOLOGY METABOLIC SYNDROME RESEARCH OpenAccess Short-termeffectoffenofibrateonC-reactive protein:Ameta-analysisofrandomizedcontrolled trials JiataoYe1,JamesNKiage1,DonnaKArnett1,AlfredABartolucci2andEdmondKKabagambe1* Abstract Background:C-reactiveprotein(CRP)ispositivelyassociatedwithriskforcardiovasculardiseaseandall-cause mortality.Somebutnotallrandomizedandnon-randomizedclinicaltrialsfoundsignificantassociationsbetween fenofibratetherapyandCRPbutthedirectionandmagnitudeoftheassociationvariedacrossstudies.Theduration oftreatment,patientpopulationsandsamplesizesvariedgreatly,andmostshort-termstudies(i.e.,≤12weeks) hadfewerthan50patients.Inthisstudywemeta-analyzedrandomizedclinicaltrialstodeterminetheshort-term effectoffenofibrateonCRP. Methods:TworeviewersindependentlysearchedPubMedandotheronlinedatabasesforshort-termrandomized clinicaltrialsthatreportedCRPconcentrationsbeforeandafterfenofibratetreatment.Ofthe81studiesexamined, 14studieswith540patientswerefoundeligible.DataforthechangeinCRPandcorrespondingmeasuresof dispersionwereextractedforuseinthemeta-analysis. Results:TheweightedmeanCRPconcentrationsbeforeandafterfenofibratetherapywere2.15mg/Land1.53 mg/L(-28.8%change),respectively.Inverse-varianceweightedrandomeffectsmeta-analysisrevealedthatshort- termfenofibratetreatmentsignificantlylowersCRPby0.58mg/L(95%CI:0.36-0.80).Therewassignificant heterogeneitybetweenstudies(Qstatistic=64.5,P0.0001,I2=79.8%).Therewasnoevidenceofpublicationbias andsensitivityanalysisrevealedthatomittinganyofthe14studiesdidnotleadtoadifferentconclusionfromthe overallmeta-analysisresult. Conclusion:Short-termtreatmentwithfenofibratesignificantlylowersCRPconcentration.Randomizedtrialsthat willrecruitpatientsbasedwithhighbaselineCRPconcentrationsandwithchangeinCRPasaprimaryoutcome areneeded.
