Methods:Thiswasarandomized,double-blind,placebo-controlledpilotstudyinamedicalICUatatertiary careuniversitycenterinGraz,Austria.Twenty-fivepatients(meanage62±16yrs)withvitaminDdeficiency [25-hydroxyvitaminD(25(OH)D)≤20ng/ml]andanexpectedstayintheICU48hourswereincludedand randomlyreceivedeither540,000IU(correspondingto13.5mg)ofcholecalciferol(VITD)dissolvedin45mlherbal oilormatchedplacebo(PBO)orallyorviafeedingtube. Results:Themeanserum25(OH)Dincreaseintheinterventiongroupwas25ng/ml(range1-47ng/ml).The highest25(OH)Dlevelreachedwas64ng/ml,whiletwopatientsshowedasmall(7ng/ml)ornoresponse(1ng/ ml).Hypercalcemiaorhypercalciuriadidnotoccurinanypatient.Fromday0today7,totalserumcalciumlevels increasedby0.10(PBO)and0.15mmol/L(VITD;P0.05forboth),whileionizedcalciumlevelsincreasedby0.11 (PBO)and0.05mmol/L(VITD;P0.05forboth).Parathyroidhormonelevelsdecreasedby19and28pg/ml(PBO andVITD,ns)overthesevendays,while1,25(OH)DshowedatransientsignificantincreaseintheVITDgrouponly. Conclusions:Thispilotstudyshowsthatasingleoralultra-highdoseofcholecalciferolcorrectsvitaminD deficiencywithin2daysinmostpat
