Casepresentation:Wereportthefirstcaseofapatientwithclearmanicepisode,aftertreatmentwith recommendeddoseofsibutramine,withoutprevioushistoryofmooddisorders,organicchangesordrug interactions,thatwasfollowedbyepisodeofdepression. Conclusion:Minimalrecommendeddoseofsibutramineinducedmanicepisodethatwasthefirstmanifestationof bipolardisorder.Themanicepisode,associatedwithsibutraminetreatment,wasinducedinapersonwithout previoushistoryofmooddisorders.Potentialrisksassociatedwiththetreatmentofobesityusingsibutraminewarn physicianstobealertnotonlytocommonandcardiovascularbutalsotopsychiatricadverseeffects.Acareful assessmentofpatient’smentalstateanddetailedpsychiatricfamilyhistoryshouldbedonebeforesibutramine treatment.Inpatientswithafamilyhistoryforbipolardisordertheuseofevenminimaldoseofsibutramineshould becontraindicated.
