Method:Thiswasaprospectivestudyinamixedsurgical/medicalintensivecareunit(ICU)ofauniversityhospital. Thepatientswere62criticallyillpatientswithaclinicalsuspicionofCIRCI.Thepatientsunderwenta250-μ gACTH test(n=67).ACTH,adrenalsteroids,substrates,andprecursors(modifiedtandemmassspectrometry)alsowere measured.Clinicalcharacteristicsincludinguseofetomidatetofacilitateintubation(n=14within72hoursof ACTHtesting)wererecorded. Results:AtthetimeofACTHtesting,patientshadseptic(n=43)ornonsepticcriticalillness(n=24).Baseline cortisoldirectlyrelatedtosepsisandendogenousACTH,independentofetomidateuse.Etomidatewasassociated withalowerbaselinecortisolandcortisol/11b-deoxycortisolratioaswellashigher11b-deoxycortisol,reflecting greater11b-hydroxylaseinhibitioninnonsepsisthaninsepsis.Cortisolincreases250mMinexogenousACTH wereassociatedwithrelativelylowbaseline(HDL-)cholesterol,andhighendogenousACTHwithlowcortisol/ACTH ratio,independentofetomidate.AlthoughcortisolincreaseswithexogenousACTH,levelswerelowerinsepsis thaninnonsepsispatients,andetomidatewasassociatedwithdiminishedincreasesincortisolwithexogenous ACTH,sothatitsuseincreased,albeitnonsignificantly,lowcortisolincreasestoexogenousACTHfrom38%to57%, inbothconditions. Conclusions:Asingledoseofetomidatemayattenuatestimulatedmorethanbasalcortisolsynthesis.However,it mayonlypartlycontribute,particularlyinthestressedsepsispatient,totheadrenald
