Purpose:TotestwhethertheeffectofvitaminConasthmaisheterogeneousovertheparticipantpopulation. Methods:Egyptianasthmaticchildrenbetween7and10yearsofage(n=60)wereincludedinthecross-over trial.Theywereadministered0.2gramsperdayofvitaminCandplaceboforseparate6-weekperiods.The variationinthevitaminCeffectontwoclinicallyrelevantoutcomeswasanalyzed:thechildhoodasthmacontrol test(C-ACT),whichmeasurestheseverityofasthmasymptoms(thescalerangesfrom0to27points,20points indicatingunsatisfactoryasthmacontrol),andFEV1.Weusedlinearmodelingtoexaminethevariationofthe vitaminCeffectinthesubgroups. Results:TheeffectofvitaminContheC-ACTwassignificantlymodifiedbyageandbaselineC-ACTlevels.Inthe childrenaged7.0-8.2yearswithabaselineC-ACTof18to19points,vitaminCincreasedtheC-ACTscoreby4.2 points(95%CI:3.3-5.3);whereasinthechildrenaged8.3-10yearswhohadabaselineC-ACTof14to15points, vitaminCincreasedtheC-ACTscorebyonly1.3points(95%CI:0.1-2.5).TheeffectofvitaminContheFEV1levels wassignificantlymodifiedbyageandexposuretodampness.Inthechildrenaged7.0-8.2yearswithnoexposure todampness,vitaminCincreasedtheFEV1levelby37%(95%CI:34-40%),whereasinthechildrenaged8.3-10 yearswithexposuretodampnessormoldintheirbedroommorethanoneyearpriortothestudy,vitaminC increasedtheFEV1levelbyonly21%(95%CI:18-25%). Conclusions:WefoundstrongevidencethattheeffectofvitaminConasthmaticchildrenisheterogeneous. Furtherresearchisneededtoconfirmourfindingsandidentifythegroupsofchildrenwhowouldreceivethe greatestbenefitfromvitaminCsupplementation.
