Methods:Weusedthreecross-sectional,multicenterdata-setsfrom2002,2005,and2008.Data-set1included diagnosticcodesfrom93%ofallpatientsreceivingtreatmentinmentalhealthcareinNorway(N=40261).Data- sets2(N=1065)and3(N=1181)includeddiagnosticcodes,patient-andclinician-reportedsleepdisturbance, anduseofprescribedhypnoticmedicationfrompatientsin8mentalhealthcarecenterscovering10%ofthe Norwegianpopulation. Results:34patientsindata-set1andnoneindata-sets2and3hadadiagnosisofinsomniaasaprimaryor comorbiddiagnosis.Indata-sets2and3,42%and40%ofthepatientsreportedsleepdisturbance,whereas24% and13%hadclinician-reportedsleepdisturbance,and7%and9%usedhypnotics.Patientsandcliniciansagreedin 29%and15%ofthecaseswherethepatientortheclinicianorbothhadreportedsleepdisturbance.Positive predictivevalue(PPV)ofclinicians’evaluationsofpatientsleepdisturbancewas62%and53%.Whenthepatient reportedsleepdisturbanceasoneoftheirmostprominentproblemsPPVwas36%and37%.Ofthepatientswho receivedhypnoticmedication,23%and29%hadneitherpatientnorclinician-ratedsleepdisturbance. Conclusion:Whenpatientsmeetthecriteriaforamentaldisorder,insomniaisalmostneverdiagnosed,andsleep disturbanceisimpreciselyrecognizedrelativetothepatients’experienceofsleepdisturbance.
