Abstract Background:Sedentarylifestyleisassociatedwithcoronaryarterydiseasebutevenshorterperiodsofphysical inactivitymayincreasecardiovascularrisk.CystatinCisindependentlyassociatedwithcardiovasculardiseaseand ourobjectivewastoinvestigatetherelationbetweenthisnovelbiomarkerandstandardizedbedrest.Researchof immobilizationphysiologyinhumansischallengingbecausegoodbiologicalmodelsareinshortsupply.Fromthe WomenInternationalSpacesimulationforExplorationstudy(WISE)westudiedmarkersofatherosclerosisand kidneyfunction,includingcystatinC,inastandardizedbedreststudyonhealthyvolunteers.Fifteenhealthyfemale volunteersparticipatedina20-dayambulatorycontrolperiodfollowedby60daysofbedrestinhead-downtilt position(-6°)24haday,finalizedby20daysofrecovery.Thesubjectswererandomizedintotwogroupsduring bedrest:acontrolgroup(n=8)thatremainedphysicallyinactiveandanexercisegroup(n=7)thatparticipated inbothsupineresistanceandaerobicexercisetraining. Results:ComparedtobaselinevaluestherewasastatisticallysignificantincreaseincystatinCinbothgroupsafter bedrest(P0.001).Glomerularfiltrationrate(GFR),calculatedbybothcystatinCandCockcroft-Gaultequation, decreasedafterbedrestwhiletherewerenodifferencesincreatinineorcreatinekinaselevels.CRPdidnotchange duringbedrestintheexercisegroup,buttherewasanincreaseofCRPinthecontrolgroupduringrecovery comparedtoboththebaselineandthebedrestperiods.Theapo-B/apo-Airatioincreasedduringbedrestand decreasedagainintherecoveryperiod.Subjectsexperiencedasmallbutstatisticallysignificantreductionin weightduringbedrestandcomparedtobaselineweightsremainedloweratday8ofrecovery.
