AkinbamiHeadFaceMedicine2011,7:10 /content/7/1/10 HEAD FACE MEDICINE REVIEW OpenAccess Evaluationofthemechanismandprinciplesof managementoftemporomandibularjoint dislocation.Systematicreviewofliteratureanda proposednewclassificationoftemporomandibular jointdislocation BabatundeOAkinbami Abstract Background:Virtuallyallthearticlesinliteratureaddressedonlyaspecifictypeofdislocation.Theaimofthis reviewwastoprojectacomprehensiveunderstandingofthepathologicprocessesandmanagementofalltypes ofdislodgementoftheheadofthemandibularcondylefromitsnormalpositionintheglenoidfossa.Inaddition, anewclassificationoftemporomandibularjointdislocationwasalsoproposed. Methodandmaterials:AthoroughcomputerliteraturesearchwasdoneusingtheMedline,Cochranelibraryand Embasedatabase.Keywordsliketemporo-mandibularjointdislocationwereusedforthesearch.Additionalmanual searchwasdonebygoingthroughpublishedhome-basedandforeignarticles.Casereports/series,andoriginal articlesthatdocumentedthetypeofdislocation,numberofcasestreatedintheseriesandoriginalarticles. Treatmentdoneandoutcomeoftreatmentwereincludedinthestudy. Result:Atotalof128articleswerereviewedoutwhich79werefoundrelevant.Ofthese,26werecasereports,17 werecaseseriesand36wereoriginalarticles.79caseswereacutedislocations,35caseswerechronicprotracted TMJdislocationsand311caseswerechronicrecurrentTMJdislocations.Etiologywaspredominantlytraumain 60%ofcasesandothercausescontributedabout40%.Ofallthecasesreviewed,only4wereunilateraldislocation. Varioustreatmentmodalitiesareoutlinedinthisreportasindicatedforeachtypeofdislocation. Conclusion:Themorecomplexandinvasivemethodoftreatmentmaynotnecessarilyofferthebestoptionand outcomeoftreatment,thereforeconservativeapproachesshouldbeexhaustedandutilizedappropriatelybefore adoptingthemoreinvasivesurgicaltechniques.
