Methods:WereviewedtherecordsofconsecutivepatientswithadvancedcancerpresentingtoOPC.Edmonton SymptomAssessmentSystem(ESAS)scoreswereobtainedattheinitialandsubsequentvisitsbetweenJanuary 2003andDecember2008.Allpatientsreceivedinterdisciplinarycareledbypalliativemedicinespecialistsfollowing aninstitutionalprotocol.Fatigueimprovementwasdefinedasareductionof≥2pointsinESASscorerelativeto thebaseline.Descriptivestatisticswereusedtosummarizepatientcharacterstics.Univariateanalyseswere performedandonlysignificantvariableswereincludedinmultivariateregressionanalysistodeterminefactors associatedwithseverityandimprovementinCRF. Results:Atotalof1778evaluablepatientswereanalyzed(medianage,59years;52%male).Themediantimebetween visitswas15days.MedianfatiguescoresontheESASwere6atbaselineand5atfollow-up.SeverityofallESASitems andlowserumalbuminwereassociatedwithfatigueatbaseline(p0.0001).Theimprovementoffatiguewasobserved in586patients(33%).Thehierarchicalmodelshowedthatfatigueimprovedovertime(b=?0.009;p=0.0009).low appetite(oddsratio[OR]=1.09perpoint;p=0.0113)andgenitourinarycancer(OR=1.74perpoint;p=0.0458)were significantlyassociatedwithimprovementoffatigue. Conclusions:CRFisstronglyassociatedwithphysicalandemotionalsymptoms.Genitourinarycancerandlowappetite atbaselinewereassociatedwithsuccessfulimprovementoffatigue.
