特发性肺纤维化患者治疗前后血清白介素8测定的临床意义 【摘要】 目的 比较特发性肺纤维化（IPF）患者与健康人血清IL-8的水平及IPF患者 治疗 前后的血清IL-8水平，得出IPF患者IL-8在血清学上的意义。 方法 采用双抗体夹心ABC-ELISA法测定30例IPF患者治疗前后及20名健康对照者的血清IL-8，同时做静态肺功能检查、心肺运动试验和血气 分析 。 结果 IPF患者血清IL-8水平显著提高（36.8±14.8pg/ml,P＜ 0.05),而健康对照组血清中IL-8浓度为10.7±5.8pg/ml。治疗后患者血清IL-8水平较治疗前显著下降（25.1±13.9pg/ml,P＜0.05)。IPF患者的肺活量（VC）、肺总量（TLC）、一氧化碳弥散能力（DLco）、最大摄氧量（VO2max）和动脉血氧分压（PaO2）显著低于正常组，治疗后上述指标显著提高；IPF患者运动后肺泡-动脉血氧分压差 〔P（A-a）O2〕显著高于正常组，治疗后运动后〔P（A-a）O2〕显著减低。而且，血清IL-8水平显示了与最大运动时的〔P（A-a）O2〕一定程度的正相关性（r=0.44,P=0.0147）。结论 IPF患者血清IL-8水平可反映疾病的活动性，并可作为疗效评价的指标。 【关键词】 特发性肺纤维化；白介素8；肺功能 The clinical significance of serum interleukin-8 detection in patients with IPF untreated and after treated for a period of time 【Abstract】 Objective To comparison the serum level of interleukin-8(IL-8)in patients with IPF untreated,those treated and healthy volunteers,and to find serological assessment of IL-8. Methods We measured IL-8 in sera of 30 patients with IPF untreated,those treated for a period of time and 20 healthy volunteers.And did the lung function tests,cardiopulmonary exercise testing(CPET)and PaO2 as well. Results IL-8 was significantly elevated in serum of patients with IPF(36.8±14.8pg/ml,P＜0.05) compared with controls(10.7±5.8pg/ml).After treated for a period of time,IL-8 was significantly decreased in serum of the IPF patients(25.1±13.9pg/ml,P＜0.05).The vital capacity(VC),total lung capacity(TLC),the lung diffusing capacity of CO(DLco),the maximal volume of oxygen uptake(VO2max)and PaO2 in patients with IPF was significantly decreased compared with controls,and then increased significantly after a period of treatment.The 〔P（A-a）O2〕in patients with IPF was significantly increased compared with controls,and then dropped significantly after a period of treatment.Furthermore,there is a positive correlation between the 〔P（A-a）O2〕in maximal exercise and serum level of IL-8 in IPF patients.
