超声诊断宫内节育器异常381例 成红宇1 琚爱霞1 刘 兰1 郜艳霞1 赵 杰2 河南省焦作市博爱县计划生育服务站 2.解放军91中心医院 河南 焦作 454450 摘要 目的：对经各种手术途径取出宫内节育器（IUD）并证实异常的381例情况进行总结分析，比较超声和X线对宫内节育器异常的诊断符合率，探讨超声对IUD异常的诊断价值。方法：所有对象均经超声和X线检查，重点观察IUD位置、数量、形状等，结合临床表现综合分析。结果：IUD异常对象共381例，其中宫内双IUD 2 例；IUD脱落23例；IUD单纯下移83例；IUD合并妊娠147例； IUD变形断裂及嵌顿126例。超声诊断相符者361例，准确率94.8%, X线相符者233例，X线诊断IUD异常准确率61.2 %。结论：超声检查IUD简单易行，无放射性，可重复，检查对象易于接受，通过超声检查不仅可分析IUD是否存在，并可以确定IUD在宫腔内的位置、数量，直观判断有无变形、移位、嵌顿、合并妊娠等。但患者子宫位置、子宫肌瘤和内膜病变、绝经年限过长、卵巢早衰等因素均会引起子宫外形和宫腔变化，对IUD位置、形态等产生系列影响，从而影响超声诊断准确率。 关键词 超声检查 宫内节育器（IUD） 分析 ［中图分类号] [文献标识码] Ultrasonography diagnostic analysis of 381 cases of abnormal IUD 【Author】CHENG Hongyu1, QU Aixia1,LIU Lan1,GAO Yanxia1, ZHAO Jie2 (1 Family Planning Service Center of BoAi County, Henan province,454450;2 The Ninety-first Central Hospital of PLA , Henan province,454003) 【Abstract】Objective Through a variety of surgical approaches to remove the intrauterine device(IUD) and confirmed abnormal 381 patients were analyzed, comparing sonography and X-ray abnormalities of the IUD diagnostic accuracy, To explore the diagnostic value of sonography in the diagnosis of abnormal IUD.
