桥本甲状腺炎中医治疗策略 　　[摘要] 桥本甲状腺炎（HT）是目前尚无法根治的自身免疫性疾病，主要的危害是导致甲状腺功能减退（甲减），形成甲状腺结节而增加肿瘤的风险，同时影响女性的生殖健康。中医称HT为瘿病，中医治疗策略在于控制病情的进展，以及改善西药治疗不能缓解的临床症状。主要是对于甲状腺功能正常的HT患者尽早应用调肝健脾补肾的中药干预，避免出现不可逆的甲减；其次是甲状腺肿大或甲状腺结节的患者采用益气祛痰化瘀法治疗，以降低癌变的风险；再就是甲减替代治疗不满意的患者、生殖健康受到影响的育龄期女性西药治疗效果不理想的患者需中医辨证治疗。中药治疗可以改善患者的自身免疫状态，降低甲状腺自身抗体，消除甲状腺肿大和结节，能够阻止或延缓HT的发生和发展以及改善临床症状，具有一定的实用价值 [关键词] 桥本甲状腺炎；瘿病；中医药；治疗策略 [中图分类号] R581 [文献标识码] A [文章编号] 1673-7210（2016）12（a）-0093-04 Strategies of traditional Chinese medicine in the treatment of Hashimoto thyroiditis JIANG Chao ZHOU Tao SHAO Yingxin▲ ZHOU Yun WANG Hong Department of Endocrinology， Huanggang Hospital of Traditional Chinese Medicine， Hubei Province， Huanggang 438000， China [Abstract] Hashimoto thyroiditis （HT） is an autoimmune disease that can not be completely cured， the main hazards of which is causing hypothyroidism， thyroid nodule and thus increasing the risk of tumor formation， meanwhile， affecting women’s reproductive health. In traditional Chinese medicine， this disease has been referred to gall disease， the focus of Chinese medicine treatment strategy is to control the progression of the disease， to prevent or delay its occurrence and development， and also to reduce the clinical symptoms which can not be alleviated by western medicine. Firstly， HT patients with normal thyroid function should be applied as soon as possible with adjustment of the liver and spleen kidney in TCM intervention， to avoid irreversible hypothyroidism. Secondly， method of regulating qi activity， invigorating the circulation of blood， and dissipating phlegm and removing stasis should be actively applied to thyroid enlargement or thyroid nodules in patients， in order to reduce the risk of cancer. Thirdly， patients having no ideal treatment with western medicine， need differentiated treatment， such as hypothyroidism patients with unsatisfactory treatment， patients of childbearing age whose reproductive health be affected and so on.
