医生、专家讲课邀请函 Speaker Invitation Letter 请在使用、签署本函前认真阅读下列使用说明 Please read these guidelines before using or signing this letter. 标准邀请函使用说明： Guidelines for Using Standard Invitation Letter 适用范围：本邀请函适用于邀请中国国内的医生、专家在葛兰素史克主办或赞助（健康组织、学术机构举办）的国内会议或活动上进行学术教育演讲。邀请任何国外讲者来中国讲课须使用该讲者所属国家或地区适用的讲者协议模板，并提交法律部审阅。 Applicability: This Invitation Letter is designed for inviting Chinese doctors / KOLs to give a speech in local meetings or events organized by GSK or by academic associations under GSK’s sponsorship. When inviting a foreign speaker, you shall use the speaker agreement template in the country where the speaker is from, and submit the agreement to China Legal Department for review. 当事人：本函以葛兰素史克公司的名义签字发出，接受邀请的医生和专家需要在下方签字表示确认和同意。供医生签署的回执页应当打印在本邀请函的同一页面上，并与其他支持文件一道保存。 Parties: This Letter shall be sent out in the name of GSK and signed by doctors/KOLs at the end of the Letter to acknowledge their confirmation and consent. The return receipt to be signed by the speaker shall be printed in the same page of the Letter and shall be filed together with other supporting documents. 签字权限和盖章：根据公司发布的《销售及推广实务指南》确定的讲课费标准，由有协议签字权限的人员代表公司签署本函，并加盖公司公章(或办事处的公章)。 Signing Authority and Chopping: This Invitation Letter shall be signed by people who have proper signing authority in line with the speak fee levels as set out by the SPPG, and shall be affixed with GSK company seal (or seal of rep office). 其他：在实际操作中，可以根据实际情况对本函标出的相关部分（特别是以不同颜色标明的部分）进行修改或选择，但前提是应确保符合公司政策和已审批的方案。对本函的使用如有任何疑问，请随时咨询法律部或财务部门。 Miscellaneous: In practice, users may make changes or selection to the relevant parts (esp. for the highlighted part hereto) according to the actual situations, provided that the changes or selection made shall be in compliance with the company policy and the approved plans/proposals.
