老年社区获得性肺炎住院患者临床分析及护理对策【摘要】 目的 分析老年社区获得性肺炎(CAP)住院患者的临床特点,并提出相应护理对策。方法 回顾2002~2008年我院收治的老年CAP患者(65岁)的临床资料,对患者原发病及病原菌特点、并发症等进行回顾性分析,并提出相应护理对策。结果 283例老年CAP患者年龄65~98岁,64.3%的患者合并基础疾病,住院期间病死率为14.1%。心力衰竭为本组患者最多见的并发症,其次休克、急性肾衰竭;针对老年肺炎的临床特点,提出加强患者的病情观察,做好基础护理及并发症的护理,给予必要的营养支持等护理对策。结论 老年CAP患者的临床表现多不典型,肺炎严重程度、并发症发生率、住院期间死亡率较高;通过对病历回顾研究,能够使我们加深对老年肺炎患者临床表现的了解,对护理患者更具有针对性。 【关键词】社区获得性肺炎;临床特点;老年患者;护理对策 Clinical analysis and nursing of elder inpatients with community acquired pneumonia ZHOU Xiao-xiang,FENG Li-qin,LIANG Yan-pin,et al.The Second Affiliated Hospital,Sun Yat-sen University Guangzhou 510630,China 【Abstract】 Objective In this article,we analyzed the clinical features of the elder inpatients with community acquired pneumonia(CAP)and put forward the nursing methods for this kind of patients.Methods We carried on the retrospective analyzed of the clinical dates,which including the primary disease,pathogens and complications,of the elder inpatients(age65 years old)with CAP during 2002~2008 in our hospital,and we put forward the nursing methods for this kind of patients.Results 64.3%of the 283 elder patients with CAP accompanied with complications and the mortality rate of these patients is 14.1%.Heart failure is the most common complication,followed by shock and acute renal failure.Because of the special clinical features of the elder inpatients with CAP,we need to strengthen the observation of the patients,provide appropriate nursing for patients? basic need and complications,and supply necessary nutritional support.Conclusion The clinical manifestations of elder inpatients with CAP is usually atypical.Their situations are often severe,while the mobility of complications and the mortality are really high.Through retrospective analyzed of the histories,we made sense of the clinical features of the elder inpatients with CAP and put forward more propose nursing methods for them. 【Key words】Community-acquired pneumonia; Clinical course; Elderly patients; Nursing st
