肌袖性房性心律失常动态心电图特点及临床意义 余芳 钟丹 舒佳 [摘要] 目的 探讨肌袖性房性心律失常(muscle sleeve atrial arrhythmias)的动态心电图特征及临床意义。方法 对11例肌袖性房性心律失常患者的动态心电图及临床资料进行回顾性分析。结果 肌袖性房性心律失常均发生在窦性心律(during sinus rhythm)的基础上，反复出现短阵房性心动过速，短阵心房扑动(atrial flutter)，短阵心房颤动(atrial fibrillation)等多种房性心律失常。各种房性心律失常常共存或交替出现，连绵不断，长期迁延。发作时间1～20min不等。诱发肌袖性房性心律失常的偶联间期(the interval of atrial premature beat)较短，几乎均呈“P-on-T”现象。结论 肌袖性房性心律失常是一组具有特征性的房性心律失常，多数患者并无器质性心脏病，药物治疗无效，射频消融术可根治。 [关键词] 肌袖(muscle sleeve) 房性心律失常(atrial arrhythmia) 动态心电图(dynamic electrocardiography) 临床 Characteristics and clinical significance of dynamic electrocardiography in atrial arrhythmia related to muscle sleeve ? Yu Fang, Zhong Dan, Shu Jia ? [Abstract]Objective: To explore the characteristics and clinical significance of dynamic electrocardiography in atrial arrhythmia related to muscle sleeve. Method: Retrospective analysis of dynamic electrocardiography and clinical features of 11 cases with muscle sleeve atrial arrhythmia. Results: Muscle sleeve atrial arrhythmia occurred during sinus rhythm and multiple atrial arrhythmias such as atrial tachycardia, atrial flutter and atrial fibrillation appeared repetitively. Various atrial arrhythmias often coexist or occur alternatively which last from 1 to 20 mintes. The interval of atrial premature triggering muscle sleeve atrial arrhythmias was comparatively short and displayed the P on T pattern. Conclusion: Muscle sleeve atrial arrhythmia is a group of atrial arrhythmia with special characteristics. Most of patient with it have no organic heart disease. Medication therapy has no effect while radiofrequency ablation can cure it radically.. [Key Words] muscle sleeve, atrial arrhythmia, dynamic electrocardiography, clinical 肌袖性房性心律失常是近年才认识到的一种特殊类型的心律失常，临床上并不少见。本文对11例肌袖性房性心律失常的动态心电图及临床意义进行回顾性分析，并对其发生机制进行讨论，现报道如下。 1 资料与方法 1.1 一般资料 选自2006年1月～2009年8月门诊及住院患者动态心电图4465例中肌袖性房性心律失常患者11例进行回顾性分析。临床资料见表1。 1.2 11例患者采用世纪今科MIC-I 12导同步心电图记录仪记录。分析监测结果，统计24h内单个房性早搏的总数，二、
