深圳市136902围产儿出生缺陷监测流行病学特征[摘 要] 目的 研究深圳市出生缺陷的发生状况及相关危险因素，为减少出生缺陷制定预防保健措施提供依据。方法 按深圳市统一的监测方案，对2007年深圳市所有医院孕20周至产后7天内（包括活产、死胎、死产、引产及产后七天内死亡）分娩的围产儿进行出生缺陷监测。结果 136，902例围产儿出生缺陷发生率为15.99‰（2189/136902）。活产畸形率为13.16‰（1790/135937）.流动人口占缺陷比例69.99%（1532/2189）.但出生缺陷的发生率以常住人口为高18.59‰。经检验P＜0.005，差异有显著性。产母文化程度低缺陷儿发生比例高。产母年龄?35岁发病率15.70‰明显高于其他年龄组，经检验 P＜0.025，差异有显著性。足月分娩的缺陷为71.45%（1564/2189）。先心+重大体表缺陷发生率为9.85‰（1349/136902）占缺陷的61.62%（1349/2189）. 缺陷儿存活占81.77%（1790/2189）。结论 非单一的体表检查缺陷发生率高。28周前的产前诊断仍需要重点加强。急需建立胎儿、新生儿内.外专科而不是一味的引产。普及孕期健康教育、开展孕前保健服务，选择最佳生育年龄妊娠、分娩，更是预防其发生的关键。 关键词：出生缺陷 发生率 流动人口 高龄产妇 产前诊断 干预 Epidemilolgy Characteristic of prenatal detection of 136,902 perinatal in Shenzhen City Yan Chunrong Gong Lin wang zhu zhen zhao guang lin Shenzhen Maternal and Child Health Institute [Abstract] Objective： Research cause of birth defect and risk factors. Reduce birth defect through institute health care and disease prevents measures based on this research. Methods： Processed by surveillance perinatal births in all Shenzhen hospital with gestational age of at least 20 weeks (including live birth, stillbirth,induced abortion and neonatal death in year 2007 according Shenzhen uniform surveillance scheme.Results： The incidence of birth defect is 15.99‰（2189/136902）. Live deformity is 13.16‰（1790/135937）. Birth defect occurs in floating population take 69.99% than local population（1532/2189）. But incidence of birth defect in local population are higher than floating population as the percentage as 18.59‰, chi square for trend was significant P＜0.005. As lower knowledge level puerpera have as higher rate birth defect occurs. Incidence rate of puerpera age?35 are much higher than other age group as the rate as 15.70‰, chi square for trend was significant P＜0.025. The rate of defect of mature puerperal are 71.45%（1564/2189）.
