中国矫形外科杂志 　2003 年 8 月第 11 卷第 16 期 　Orthop J Chin ,Vol11 ,No16 　August 　2003 加页1 ·论　著· 改良 Ka pandji 手术在治疗尺桡骨 远端陈旧性骨折中的应用 蔡培华 　何耀华 　张先龙 　于仲嘉 摘 　要 　目的 :观察应用改良 Kapandji 手术治疗尺桡骨远端陈旧性骨折的治疗效果。方法 : 自 1996 年 9 月～2001 年 9 月共收 治尺桡骨远端陈旧性骨折 15 例。采用改良 Kapandji 手术方法以矫正骨折畸形、改良前臂旋转功能。根据术后前臂旋转活动范围、 截骨区旋转痛、冲击痛以及手腕部功能和负重情况有无改善等评价其临床效果。结果 : 13 例患者获良好功能恢复 ,优良率为 866 % 。无感染、骨不连等发生。结论:改良 Kapandji 手术是治疗尺桡骨远端陈旧性骨折伴有前臂旋转痛和功能受限的疗效好、后 遗症少的手术方法。 关键词 　骨折 ; 　脱位 ; 　截骨 ; 　关节融合 ( ) 中图分类号　R683. 42 　　　文献标识码 　A 　　　文章编号 　1005 - 8478 2003 16 - 加页 - 02 1 Modif ied Kapandji Procedure on the Obsolete Fracture of the Distal End of Radius and Ulna ∥CA I Peihua , HE Yaohua , ZHA N G Xianlong , et al Depart ment of Orthopedics , the Sixth People ’s Hospital , S hanghai Jiaotong U niversity , S hanghai 200233 Abstract 　Objective : To observe the modified Kapandji operative effect on the obsolete fracture of the distal end of radius and ulna. Method : Fifteen cases with the fracture of distal end of the radius and ulna were reviewed from Sep. 1996 to Sep. 2001. Used the modified Kapandji procedure to correct the fracture deformity and improve the rotation function of forearm. To evaluate the clinical effects according to the range of forearm ,rotation and impaction pain on the osteotomy area ,and the function or load transmission of wrist joint. Result :A mong the 15 cases ,13 cases were excellent in function ,and excellent rate was 866 % ,no infection and nonunion. Conclusion : It is an effec tive and less complicative operation to be adopted modified Kapandji procedure on the fracture of the distal end of the radius and ulna.
