四肢骨折合并血管损伤21例（Limb fracture complicated with vascular injury in 21 cases） Limb fracture complicated with vascular injury in 21 cases Update Date: 2011-07-22 Chiu Chiu, Cai Zhiying, Lin Bendan, Zhong Zhigang Since 1985, 78 cases of main vascular injuries in limbs have been treated in our hospital, including 21 cases of limb fracture and vascular injury, accounting for 27%. clinical data 1. general data: this group is male 17, female 4, age 14~60 years old, average 26.2 years old. Cause of injury: 10 cases of crush injury, 6 cases of traffic accident injury, 3 cases of gunshot wounds, 2 cases of machinery injury. Fracture and vascular injury: proximal humerus fracture combined with brachial artery injury in 1 cases of supracondylar humerus with brachial artery injury in 4 cases of fracture of distal femoral fracture with femoral artery injury in 3 cases, distal femur fracture with popliteal artery injury in 2 cases, proximal tibia fracture with popliteal artery injury in 8 cases, fracture of tibia and fibula tibia before and after artery injury in 3 cases. Injury time to surgery: 6 h within 7 cases, 6~12 h in 11 cases, 12 h in 3 cases, 7 d in length. 2. treatment and outcome: direct microvascular anastomosis of artery injury in 15 cases, artificial vascular graft in 2 cases, saphenous vein transplantation in 3 cases, vascular exploration and lysis in 1 cases. Direct anastomosis of 15 arteries, 14 patency, patency rate of 93%. 4 limbs were amputated and amputation rate was 19%. All the fractures were treated by open reduction, internal fixation with plate or Kirschner wire, and 11 cases underwent fascia decompression. Hyperbaric oxygen therapy was used in 15 cases. There was no death in the whole group. Discussion Limb fracture combined with vascular injury is not rare, early diagnosis, timely and excellent surgical treatment is the key to cure the patient. 1. patients with limb trauma should be highly vigilant for the possibility of concurrent vascular injury.
