扁桃体切除术中两种扁桃体剥离方式探析 　【关键词】 慢性扁桃体炎；扁桃体切除术；剥离法 　　［摘要］目的探讨扁桃体切除术中扁桃体剥离的两种方式（剥离法，分剥法），比较两种方式治疗效果的差异。方法慢性扁桃体炎病员226例，病变扁桃体452侧，按单侧计分成两组，分别采用剥离法、分剥法剥离并切除扁桃体，比较不同方式的剥离时间、手术时间及术中出血量。结果分剥组剥离时间（4.63±0.48） min，手术时间（6.0±0.58） min，出血量（8.8±2.9） ml；剥离组剥离时间（9.3±0.95） min，手术时间（11.85±1.07） min，出血量（14.5±3.7） ml，与分剥组比较相差非常显著（P0.01）。结论慢性扁桃体炎是耳鼻咽喉科的常见疾病，扁桃体切除是常用的治疗方法，在扁桃体剥离中采用分剥法具有明显的优势。 　　［关键词］ 慢性扁桃体炎；扁桃体切除术；剥离法 　　Study on two kinds of shelling methods of tonsillectomy 　　Abstract: objectiveTo study two kinds of methods of tonsillectomy(shelling method, separatingshelling method) , compare two kinds methods of tonsillectomy in Surgical operation time，shelling time and bleeding amount．MethodsDuring the period from 2000 to 2006, 226 patients were operated tonsillectomy for chronic tonsillitis, the total sick tonsils were 452, all tonsils were divided into 2 groups according to single side tonsil, there were 226 tonsils in the first group who were operated the tonsillectomy by shelling, there were 226 tonsils in the second group who were operated the tonsillectomy by separatingshelling, two methods were examination by surgical operation time analysis, shelling time analysis and bleeding amount analysis．Resultsseparating shelling group: separating and shelling 4.63 minutes, surgical operation 6 minutes, bleeding 8.8 milliliters; the shelling group: shelling 9.3 minutes, surgical operation 11.85 minutes, bleeding 14.5 milliliters．One was significantly different from the other methods by statistics testing (Plt;0.01). Conclusion Chronic tonsillitis is the familiar disease of otolaryngology, tonsillectomy is still an extremely common treatment method, separatingshelling method has two important advantages: a little bleeding and shorter periods during the operations. 　　Keywords: chronic tonsillitis； tonsillectomy； dissection method 　　慢性扁桃体炎是耳鼻咽喉科的常见病，扁桃体切除术（剥离法）是治疗慢性扁桃体炎的常用方法，扁桃体剥离的方法很多，国内多数医生采用扁桃体剥离器剥离扁桃体(剥离法)。也有用大号弯血管钳分离、剥离扁桃体者［12］，为了便于区别于传统的“剥离法”，此法可简称“分剥法”。现将我科于2000年7月至2006年7月收治的慢性扁桃体炎患者226例扁桃
