自体游离骨膜移植修复家兔关节软骨缺损形态学探究 　【摘要】 ［目的］观察并分析移植自体游离骨膜修复家兔关节软骨缺损的形态学变化过程.［方法］在18只中国家兔双侧膝关节股骨髌面上制作6.0mm×3.0mm大小的全层关节软骨缺损模型，行胫骨内侧面骨膜片移植，利用显微镜及图像分析技术观察股骨髌面关节软骨缺损修复的形态学变化.［结果］术后第4周时，自体游离骨膜组可见明显的软骨陷窝及同源细胞群，第12周时，细胞密度降低，基质增多，接近正常透明软骨的特征.自体游离骨膜移植组在术后第4，8，12周时平均单个软骨细胞面积与对照组相比较均有显著性差异.［结论］移植自体游离骨膜可修复关节软骨缺损，其过程可分为细胞分化增殖期、幼稚软骨形成期及软骨改造塑形期等3个阶段. 【关键词】 骨膜 移植 自体 兔 Study of morphological changes in repairing articular cartilage defects with transplanting autologous free periosteum ABSTRACT:OBJECTIVETo observe and analyze the process of morphological changes in repairing articular cartilage defects with transplanting autologous free periosteum.METHODSUsing the models of 6.0mm×3.0mm of full thickness of articular cartilage defects on the patellar surface of femur established in 18 cases of knee joint of Chinese rabbits, the autologous free periosteum was transplanted into the facies medialis tibiae, and the morphological changes were observed in repairing articular cartilage defects with transplanting autologous free periosteum on the patellar surface of the femur by microscope and image analysis.RESULTSDistinct cartilage hollows and homologous cell mass were revealed when 4 weeks after operation. The characteristics similar with normal hyaline cartilage were revealed by decreasing of cell density and increasing of cell matrix when 12 weeks after operation in the transplanting group of autologous free periosteum. There were significant differences between the transplanting group of autologgous free periosteum and control group on the single cartilage cell area when 4 weeks, 8 weeks and 12 weeks after operation respectively.CONCLUSIONThe transplantation of autologous free periosteum could repair articular cartilage defect, and its course can be devided into three periods, including cell differentiation and proliferation, formation of immature cartilage and the cartilage transformation and shaping.
