Methods: The target population of 103 patients with ischemic stroke was selected in the community and managed continuously. Their index of dietary, blood pressure self-concern, score of symptoms and the difference of other indicators were compared by themselves before and after management. Results: With the continuity management, their dietary status was improved obviously, the blood pressure self-concern degree increased and the score of symptoms decreased, all of which had obvious difference (P0.05). Conclusion: The continuity management has the certain effect to improve the cognitive behavior of the community patients with ischemic stroke.
