外固定支架和钢板内固定治疗成人桡骨远端不稳定型骨折临床探究 　　[摘要] 目的 探讨比较外固定支架与钢板内固定治疗成人桡骨远端不稳定型骨折的临床疗效和安全性。 方法 将我院骨科收治的桡骨远端不稳定型骨折164例患者随机分为外固定支架组和钢板内固定组，分别采用闭合复位外固定支架进行固定和切开复位钢板内固定治疗进行固定，比较两组的Gartland-Werley临床功能评分、骨折复位情况和不良反应的发生情况。 结果 钢板内固定组的优级比例为58.4%，显著高于外固定支架组的优级比例43.9%（P 0.05）。 结论 钢板内固定可以恢复关节面的平整，实现骨折处的良好对位，对恢复掌倾角和尺偏角效果较好，对预防桡骨短缩、恢复腕关节功能具有良好的临床疗效，患者术后生活工作能力显著增强，临床疗效优，患者和家属的满意度高，值得临床进一步推广使用。 [关键词] 外固定支架；钢板内固定；桡骨远端不稳定型骨折 [中图分类号] R687.3 [文献标识码] B [文章编号] 1673-9701（2013）34-0043-04 Clinical investigation of treatment of adult unstable distal radius fracture with external fixation and plate internal fixation WU Lizhi1 HONG Huaxing1 GU Shilin1 YI Liqi1 CAI Qingkang2 1.Department of Hand and Foot Surgery， Taizhou Enze Medical Center Luqiao District Hospital in Zhejiang Province， Taizhou 318050， China； 2.Department of Orthopedics， Taizhou People’s Hospital in Jiangsu Province， Taizhou 225300， China [Abstract] Objective To compare the clinical effects and safety of external fixation and plate internal fixation treatment on adult unstable distal radius fracture. Methods The 164 qualified patients were divided as external fixation group and plate internal fixation randomly. The patients were treated with closed reduction and external fixation for external fixation group， open reduction and internal fixation for plate internal fixation group， respectively. The clinical effects of Gartland-Werley， fracture reduction and adverse reactions were evaluated and compared， respectively. Results After treatment， the excellent ratio of plate internal fixation group （58.4%） was significantly higher than that of external fixation group （43.9%） （P 0.05）. Conclusion Plate internal fixation can restore the articular surface， good alignment of the fracture and wrist joint function. Besides， it also can prevent the loss of palmar tilt angle， ulnar deviation angle and radius length and it’s worthy to be widely spread in clinical condition.
