手术治疗高位腰椎间盘突出症的疗效观察 研究生：刘选泽 专业：外科学 导师：尚显文 【摘要】目的 观察手术治疗高位腰椎间盘突出症的临床疗效，选择最佳手术方 式。方法 搜集我院2011 年1 月-2014 年2 月接受开放性手术治疗的14 例高位 腰椎间盘突出症的患者的临床资料，观察治疗前后患者神经功能改善情况，术后 采用 JOA 评分法（29 分法）对患者进行评分而判定手术疗效。结果 手术平均 时间3 小时，所有患者术后均未发生感染等并发症。4 例患者术后72h 内症状得 到不同程度的缓解，1 例患者术后症状无缓解，术后随访1—12 个月，平均7 个 月,术后1 年症状缓解优良率为85.71%，随访时根据JOA （29 分法）评分，患者 总体的评分从术前的平均11.50 分提高到末次随访时的平均24.79 分。结论 手 术治疗高位腰椎间盘突出症具有良好的效果，能够有效改善患者的生活质量。 【关键词】手术;高位腰椎间盘突出症; CT 及MRI;疗效 英文摘要 Effects of surgical treatment of upper lumbar disc herniation Liu xuanze Guiyang Medical College,Guiyang 550004 【Abstract 】 Objective: To investigate the clinical effects of surgical treatment of upper lumbar disc herniation , and choose the best surgical approach . Method: Betweem January 2011 to February 2014 ,a total of 14 cases with upper lumbar hermination underwent open surgical treatment , the clinical outcome before and after surgery was evaluated by system of Japanese Orthopaediac Associatiion Scores for Assessment of Low Lumbar Pain (JOA) . Results:The mean operate duration was 3 hours , infections and other complications did not occur in all patients after surgery . Four cases of patients within 72h symptoms varying degrees of ease , one case of postoperative symptom-free remission , patients were followed up 1-12 months , an average of 7 months , 1 year after remission rate was 85.71% and the excellent follow-up when according to JOA (29 points system ) score , patient overall score improved from an average of 11.50 points preoperatively to an average final follow-up 24.79 points.
