手术治疗胸椎黄韧带骨化症疗效的影响因素分析 专业：外科学 研究生：于海 导师：尚显文 【摘要】目的：探讨影响胸椎黄韧带骨化症患者手术疗效的相关因素，找出评 估患者预后的术前指标，为临床工作提供参考。方法：搜集我院近三年手术治疗 的 25 例胸椎黄韧带骨化症患者的临床资料进行回顾性分析，观察患者治疗前后 神经功能恢复情况，并进行随访 24 个月，平均 15 个月。术前术后采用 JOA 评分 法对患者进行评分而判定手术疗效。疗效评价选用患者的术后改善率，改善率 =(术后 JOA 评分一术前 JOA 评分) ／(11一术前 JOA 评分)×l 00％，术后 JOA 评 分为最后的随访结果。 （1）研究患者术后改善率与椎管面积残留率(椎管和脊髓的区域在最大压缩管有 效面积 /椎管发育区域 X 1 OO ％)、术前病程时间长短检测指标的相关性，判 断影响手术疗效的指标。 （2）所有病例将根据是否合并其他脊髓病变（颈椎、胸椎后纵韧带骨化，胸椎 间盘突出症，脊髓型颈椎病，腰椎管狭窄症）分为两组，一组是合并脊柱其他病 变，另一组是单纯性胸椎黄韧带骨化组，手术前后的相关数据进行比较，根据测 试结果来验证术后改善率差异是否涉及合并其他脊髓病变。 （3）将病例患者术前 MRI 图像根据T2 相内受损节段有无 T2 相增高分为两组， 两组术后改善率的差异，判断 T2 相术前病变节段高信号是否可以影响手术效果。 结论：患者有无其他脊柱病变、病变脊髓节段数、年龄与手术预后没有明显关系。 而与手术预后关系显著椎管面积残余率、术前病程、术前病变节段 T2 相高信号 可以作为判断患者预后的指标。 【关键词】手术治疗：黄韧带骨化症：预后，胸椎：影响因素 英文摘要 Factors surgery thoracic OLF herniation analysis Tutor shang Xian Wen Graduate students Yu Hai Objective: To investigate the effect of treatment of ossification of yellow ligament related factors.
