精品论文 参考文献 保护性肺通气治疗重症胸部创伤并ARDS的疗效观察 简邦浩 邵阳市中心医院 湖南邵阳 422000 【摘 要】目的 探讨保护性肺通气治疗法在重症胸部创伤并急性呼吸窘迫综合征（ARDS）临床治疗中的应用效果。方法 收集2014年1 月至2016 年1 月我院收治的重症胸部创伤并ARDS患者55 例，采用随机数字法分为两组，对照组（n=27）采用常规通气治疗法进行治疗，观察组（n=28）采用保护性肺通气治疗法进行治疗，比较两组患者治疗前后肺部通气功能、治疗效果和呼吸机辅助呼吸时间。结果 观察组患者治疗后肺部通气功能明显优于对照组，Plt;0.05；观察组患者治疗后PaO2/FiO2为394.25plusmn;36.22 mmHg，对照组为336.18plusmn;39.83 mmHg，Plt;0.05；观察组患者治疗有效率为89.28 %，对照组为74.07 %，Plt;0.05；观察组患者呼吸机辅助呼吸时间为4.41plusmn;1.77 d，对照组为6.49plusmn;1.62 d，Plt;0.05。结论 保护性肺通气治疗可有效改善重症胸部创伤并ARDS患者肺部通气功能，对于提高患者的治疗效果，减少呼吸机辅助呼吸时间均具有积极作用，值得推广应用。 【关键词】保护性肺通气；重症胸部创伤；ARDS 【Abstract 】 objective to explore the protective lung ventilation therapy in severe chest trauma and acute respiratory distress syndrome（ARDS）is the application of the clinical treatment effect.Methods between January 2014 and January 2014 our hospital 55 cases of severe chest trauma and ARDS patients，divided into two groups by random number method，the control group（n = 27）with normal ventilation therapy treatment，observation group（n = 28）with lung protective ventilation therapy treatment，compared two groups before and after treatment in patients with pulmonary ventilation function，treatment effect and the breathing machine auxiliary time to breathe.Results observation group after treatment in patients with pulmonary ventilation function was better than control group，P lt; 0.05; Observation group after treatment in patients with PaO2 / FiO2 was 394.25 + / - 36.22 mmHg and 336.18 + / - 39.83 mmHg，the control group（P lt; 0.05; Observation group of patients treatment effective rate of 89.28%，74.07%，the control group（P lt; 0.05; Observation group of patients with ventilator breathing time was 4.41 + / - 1.77 d，6.49 + / - 1.62 d，the control group（P lt; 0.05）.Conclusion lung protective ventilation therapy can effectively improve pulmonary ventilation function in patients with severe chest trauma and ARDS，to improve the therapeutic effect of patients，reduce ventilator breathing time all has
